Group Facilitator - Nursing Home (QMHS III)

About the Role

Attain Behavioral Health is seeking a Group Facilitator to lead engaging, person-centered Therapeutic Behavioral Services (TBS) groups within our partner nursing homes. This unique role blends mental health support, therapeutic engagement, and relationship-building with residents who benefit greatly from structured emotional and behavioral health programming.

What You'll Do

  • Facilitate meaningful TBS groups that support emotional health, coping skills, motivation, and quality of life for nursing home residents

  • Adapt group content to meet the cognitive and emotional needs of older adults, including those with medical complexities or mild cognitive impairment

  • Collaborate closely with facility staff and our on-site clinical team to ensure residents receive consistent, compassionate care

  • Complete documentation (group notes, progress updates, Medicaid paperwork) accurately and on time

  • Support resident engagement by encouraging participation, fostering connection, and creating a safe, respectful group environment

  • Participate in ongoing supervision and professional development as part of a supportive and collaborative behavioral health team

Qualifications

  • Must hold one of the following: QMHS III, LSW, LPC, or equivalent Ohio-recognized behavioral health credential

  • Experience facilitating behavioral health or substance-use groups

  • Experience working with older adults, long-term care, or medically complex populations strongly preferred

  • Strong communication skills, empathy, and an ability to connect with residents patiently and respectfully

  • Able to work independently in off-site facilities

  • Reliable transportation required

  • Must meet Ohio Medicaid and background check requirements

Preferred Skills & Attributes

  • Warm, compassionate approach with a genuine interest in the wellbeing of older adults

  • Experience with EHR systems (training available)

  • Trauma-Informed Care training or willingness to obtain within 90 days

  • Comfortable in nursing home environments and able to walk/stand as needed

  • Flexible, adaptable, and comfortable traveling between locations
