The Department of Behavioral Health and Developmental services support individuals by promoting recovery, self-determination, and wellness in all aspects of life. DBHDS operates 12 facilities: 8 behavioral health facilities for adults, a training center, a psychiatric facility for children and adolescents, a medical center, and a center for behavioral rehabilitation. State facilities provide highly structured, intensive services for individuals with mental illness, intellectual disability, or need substance use disorder services. DBHDS is committed to a life of possibilities for all Virginians. This position will work at Southern Virginia Mental Health Institute, a 72-bed Joint Commission accredited State Mental Health Facility, located in Danville, VA.
The Social Work Department is seeking a Case Manager whose responsibilities will focus on completing tasks relevant to the State Hospital 30-day LOS (Length of Stay) pilot initiative. We are seeking an energetic, clinically skilled, organized individual to provide a range of casework activities related to a comprehensive needs assessment involving each patient's discharge planning process for psychiatric patients in an inpatient setting. This position will serve as a Case Manager, providing an array of services including Case Management, psychosocial assessments, and social work services on the unit and may serve on department and facility committees. The Case Manager will coordinate the discharge planning needs of assigned patients, with treatment team and CSB staff.
The following Knowledge, Skills and Abilities are required of this position:
Fundamental knowledge of the principles, practices, services and techniques of case management.
Demonstrated ability and skills to assist in coordinating resources for patients.
Ability to perform duties at a fast pace with frequent change.
Ability to interact with other professionals from various disciplines.
Ability to maintain safety precautions.
Ability to form a working relationship with patients and families; determine and select services; and communicate effectively, verbally and in writing.
Must be able to work a flexible schedule.
Must be able to provide thorough documentation in an electronic health record (EHR).

Minimum Qualifications

A Bachelor's degree in Social Work (BSW) from an accredited school of social work with case work/group work experience.
A valid driver's license is required and must remain valid and in good standing throughout employment.
This position will also require:
Ability to work in a secure and locked environment with individuals who are experiencing acute symptoms of mental illness and/or additional forensic/legal involvement. Fundamental knowledge of the principles, practices, services and techniques of case management.
Ability and skills to assist in coordinating resources for patients.
Ability to perform duties at a fast pace with frequent change.
Ability to interact with other professionals from various disciplines.
Ability to maintain safety precautions.
Ability to form a working relationship with patients and families; determine and select services; and communicate effectively, orally and in writing.

Additional Considerations

Experience in collaboratively working with various community social service and behavioral health agencies.
Experience as a Case Worker in a psychiatric healthcare facility, acute care, long-term or correctional setting.
Veterans and people with National Service.
Bilingual fluency in Spanish and English.
