SUMMARY OF JOB RESPONSIBILITIES:
The Behavioral Health Social Work Team Lead serves as a clinical leader in the Behavioral Health Department to ensure that each patient receives quality, individualized care that is proactive and coordinated among the care delivery team and that the patient is attaining their clinical milestones and patient flow.
The Team Leader is responsible for assisting with the oversight of daily operations of the Behavioral Health Unit. The Team Leader is responsible for the development and coordination of activities that promote clinically effective and efficient patient care. Plans, organizes and directs patient care activities under the direction of BHI Quality Operations Program Manager. Collaborates with other health care team member and community organizations to provide safe efficient quality care. Works with the BHI Quality Operations Program Manager, in maintaining unit operations and supervises assigned personnel.
QUALIFICATIONS:
EDUCATION:
Required: Graduate Degree, Independently Licensed or working towards obtaining within one year of hire.
Specialty Major: Mental Health and/or Chemical Dependency
Preferred: Graduate Degree, Independently Licensed with Supervision Designation.

Current license in the State of Ohio
EXPERIENCE:
  • Previous experience in the behavioral health field
  • Prior knowledge and experience with this service line

KNOWLEDGE AND SKILLS:
  • Ability to analyze clinical, financial and social situations, identifying problems and alternative courses of action.
  • Working knowledge of Clinical Milestones and its impact on patient throughput
  • Willingness to be flexible, resourceful, and creative in problem solving.
  • Ability to act independently and offer suggestions and new ideas for improving performance and operations.
  • Ability to guide and direct the team to assure optimal outcomes. Clinical expertise in specialty area.
  • Ability to organize and present information clearly and concisely.
  • Excellent interpersonal skills in order to effectively communicate with physicians, patients, families and other members of the interdisciplinary team.
  • Creative problem solving and critical thinking.
  • Excellent customer service skills.

PRINCIPAL JOB ACCOUNTABILITIES
Responsible for timely assignment and facilitation of admissions, transfers and discharges based on established milestones. Anticipates and intervenes to assure clinical milestones are met, including rounding with physician to communicate achievement to the physician/consultant. Participates/facilitates with biopsychosocial needs of patients. Helps to establish daily patient goals and help to eliminate barriers in meeting those goals. Maintains working knowledge of proper patient type and consults with nursing and physicians for disposition planning/barriers to discharge as needed. Supports implementation of new processes to improve patient care Identifies performance improvement opportunities and offers suggestions for improvement. Role models clinical excellence and demonstrates current technical and therapeutic skills. Assists in maintaining appropriate staffing assignments (scheduling). Assists with orientation, competencies, and continuing education for staff In collaboration with the BHI Quality Operations Program Manager, enforces code of conduct and policies using effective and constructive communication skills. Assists with peer interviewing as requested. Active participant in service recovery through investigation and intervention. Performs chart audits for accuracy and completeness. Works with other departments to coordinate patient care. Facilitates unit cleanliness and order and participates in environmental rounds. Evaluates the care provided by other members of the team. Facilitate daily shift huddles Oversight of social work interns
