Overview

Reporting to the Executive Director, Brooklyn Health Home, the Population Health Program Associate supports care management referrals, member transitions, and network performance.


Responsibilities
  • Conduct telephonic assessments with community members and providers to direct referrals to Brooklyn Health Home and CBOs for care management enrollment
  • Monitor and support a small caseload to ensure service connections
  • Perform compliance monitoring of care management agencies, including bi-annual chart reviews and quality audits
  • Complete behavioral health/social service assessments for service referrals
  • Support initiatives for individuals in the criminal legal system
  • Track and follow up with care management staff on referral responsiveness
  • Evaluate and assign Health Home referral requests from members, providers, and community stakeholders
  • Communicate with managed care organizations, providers, hospitals, FQHCs, and community-based organizations to facilitate member outreach and engagement

This position works collaboratively with CSO staff, Health Home members, care management providers, and community partners to ensure quality care coordination.


Qualifications
  • Bachelor’s degree 
  • Minimum of 2 years related experience in healthcare, social services or community health
  • Familiarity with Home Health, community-based programs and populations health initiative sand experience engaging and evaluating patient/clients
  • Experience with programs for people involved in the criminal legal system
  • Strong organizational and verbal/written communication skills.
  • Ability to thrive in a fast-paced, highly collaborative environment.

Pay Range
USD $50,000.00 - USD $55,000.00 /Yr.
