Job description

POSITION: Counselor

REPORTS TO: Program Director

REQUIREMENTS: Bachelor's Degree

Criminal check approval (NYS/FBI)


GENERAL RESPONSIBILITIES:

Provision of restorative services to clients in unit.


PRIMARY TASKS & RESPONSIBILITIES:

1. Maintain primary responsibility and accountability for assigned caseload of clients.

2. Perform functional assessments of clients to determine service needs.

3. Develop service plans, with client input, which address functional assessment needs.

4. Assist clients in acquiring skills needed to live independently by providing restorative services on service plan.

5. Develop discharge goals with client from day of admission.

6. Document all interactions with clients through progress notes written with a frequency determined by Program Director.

7. Attend and participate in program meetings.

8. Attend weekly supervision with Program Director.

9. Participate in agency recreational activities for clients.

10. Request urine from clients for toxicological analysis as a treatment tool in providing substance abuse services.

11. Accompany clients to hospital emergency rooms during a crisis.

12. Accompany clients to appointments as needed and assigned by Program Director.

13. Attending trainings assigned by Program Director.

14. Participate in agency annual Anti-Sexual Harassment training.

15. Supervise medication as scheduled by Program Director.

16. Attend community meetings.

17. Facilitate groups assigned by Program Director.
