Job description

Bilingual Pediatric Case Manager (Spanish Required)
Location: Los Feliz, CA
Schedule: Monday-Friday | 8:30 AM - 5:00 PM
Work Arrangement: Hybrid after training (3 days in office, 2 days remote)
Are you passionate about helping children and families navigate their healthcare? We're seeking a Bilingual (Spanish) Pediatric Case Manager to join a collaborative team dedicated to improving pediatric health outcomes through patient outreach, education, and care coordination.
This is an excellent opportunity for someone who enjoys building relationships over the phone, staying organized, and ensuring families receive the care and resources they need.
Responsibilities
  • Schedule follow-up appointments for pediatric patients.
  • Conduct outbound calls to ensure patients remain compliant with recommended well-child visits, immunizations, lead testing, and preventive care.
  • Monitor patient data and track compliance with required pediatric visits during the first 16 months of life.
  • Educate patients and families on preventive healthcare and available resources.
  • Send health education materials and follow up with families as needed.
  • Coordinate care and communicate with patients to ensure timely completion of appointments and screenings.
  • Maintain accurate documentation and update patient information in electronic systems.
  • Utilize computer systems to track patient outreach and compliance metrics.
  • Perform additional care coordination duties as assigned.

Qualifications
  • Fluent in both English and Spanish (required).
  • Comfortable spending a significant portion of the day on the phone.
  • Strong customer service and communication skills.
  • Excellent organizational skills and attention to detail.
  • Computer proficiency and the ability to navigate multiple systems.
  • Experience in pediatrics, case management, behavioral health, or patient coordination is preferred but not required.
  • Medical Assistant certification is not required. Comprehensive case management training will be provided.

Schedule & Training
  • Monday through Friday, 8:30 AM - 5:00 PM.
  • Training will be conducted fully onsite for approximately one month, depending on individual progress.
  • After successful training, the schedule transitions to a hybrid model with 3 days onsite and 2 days remote each week.
