Responsibilities:
  • Hospice visits daytime visits
  • Provide accurate and timely documentation
  • Assess and provide patient and family/caregiver education
  • Perform other duties as assigned by the registered nurse
  • Other visit as needed based on your availability including routine and PRN, visits, discharges, Deaths, CC, etc.
  • Qualifications:
  • Demonstrated compassion and commitment to patientcentered care
  • Active LVN license in the state of California
  • Valid California driverโ€™s license and reliable transportation
  • Minimum 1 year of hospice nursing experience; strong understanding of hospice philosophy and endoflife care
  • Prior experience in nursing or related medical fields
  • HomecareHomebase/Pointcare EMR: 1 year (Preferred)
