About the Role:
Cruz Home Health Care LLC is seeking a compassionate, skilled Registered Nurse to join our dedicated home health team in Ontario, CA. This is an incredible opportunity to make a meaningful difference in patients' lives by delivering personalized, high-quality care right in the comfort of their homes. Responsibilities:
  • Conduct comprehensive patient assessments and develop individualized care plans for home health patients
  • Administer medications, wound care, IV therapy, and other skilled nursing treatments in the home setting
  • Monitor and evaluate patient health status, documenting changes and reporting to physicians as needed
  • Educate patients and family members on disease management, medication adherence, and home safety
  • Coordinate care with interdisciplinary team members including physical therapists, social workers, and physicians
  • Maintain accurate and timely clinical documentation in compliance with Medicare, Medicaid, and agency standards
  • Ensure patient care aligns with OASIS assessments and home health plan of care requirements
Requirements:
  • Active and unrestricted Registered Nurse (RN) license in the state of California
  • Minimum 1 year of clinical nursing experience; home health experience preferred
  • Knowledge of OASIS documentation, Medicare/Medicaid home health regulations a plus
  • Valid California driver's license, reliable transportation, and current auto insurance
  • Strong clinical assessment, critical thinking, and time management skills
  • Excellent communication and patient education abilities
  • Current CPR/BLS certification required
  • Willing to travel to Los Angeles, San Bernardino, and Riverside Counties
