Summary:

Serves as a Licensed Vocational Nurse for a Skilled Nursing Facility. Professional Experience Requirements: For Skilled Nursing Facility- A minimum of one (1) year of experience in a Skilled Nursing Facility is required. Responsibilities: Performs designated job duties under the direction and supervision of a Registered Nurse while maintaining the LVN scope of practice. Participates in the development of an individualized interdisciplinary plan of care. Implements the individualized interdisciplinary plan of care according to biophysical, psycho-social, and age-specific needs. Monitor patient/resident outcomes and report abnormal findings to the Assistant Clinical Manager/Supervisor. Administers specified medications according to established standards and policies. Documents patient's status according to the unit-specific procedure. Maintains educational requirements as required by the California Board of Vocational and Psychiatric Technicians. Documents according to federal and state regulations. Performs other duties as assigned while maintaining the LVN scope of practice. Education Requirements: Graduate of an accredited LVN school of nursing. Licensure/Certification Requirements: Current CA State LVN license Professional Experience Requirements: Current BLS certification. Negative TB Test. Valid Drivers license.
