HHA - Part Time - 2nd Shift

Location: Jamestown, NY
Schedule: Every other weekend minimum, with additional shifts as available
Salary Range: Position starts at $18.65/hour
Shift Differential: +$1.50/hour for 2nd shift


Join Our Team at Hultquist Place

Lutheranโ€™s Hultquist Placeโ€”an Assisted Living Program (ALP) operating in coordination with Licensed Home Care Services Agency (LHCSA) standardsโ€”is seeking a compassionate and dependable Certified Home Health Aide (HHA) to join our 2nd shift team, working 2pm - 10pm.

Our HHAs play a vital role in supporting residentsโ€™ independence, dignity, and safetyโ€”especially during evening hours when attentiveness and responsiveness matter most.

Position Summary:

Under the supervision of the Director of Patient Services (DPS) and licensed nursing staff, the HHA provides personal care and supportive services in accordance with each residentโ€™s individualized care plan and applicable ALP/LHCSA regulations. This role includes assisting with self-administration of medications (following facility training and protocols), overnight monitoring, and timely communication of any changes in resident condition.

Qualifications:

  • High School Diploma or equivalent required
  • Valid New York State HHA Certification required (or equivalent approved training/exam)
  • OR Certified Nursing Assistants (CNAs) willing to complete HHA conversion testing are encouraged to apply
  • Prior experience in assisted living, home care, or long-term care preferred
  • Medication assistance experience preferred (training provided)
  • Basic computer skills for EMR documentation
Key Responsibilities:Resident Care & Support
  • Provide hands-on assistance with activities of daily living (ADLs), including bathing, dressing, toileting, grooming, and mobility
  • Promote resident independence, dignity, and choice in all interactions
  • Conduct routine overnight rounds to ensure resident safety, comfort, and well-being
  • Respond promptly to call lights and resident needs
Medication Assistance
  • Assist residents with self-administration of medications in accordance with ALP guidelines and facility policies
  • Follow all medication assistance protocols, including proper documentation and reporting
  • Immediately report any medication concerns, refusals, or discrepancies to licensed nursing staff
  • Do not interpret physician orders or independently administer medications outside of scope
Monitoring & Reporting
  • Observe, document, and report changes in resident condition, behavior, or needs to the Charge Nurse/DPS
  • Accurately complete documentation in the electronic medical record (EMR) in a timely manner
  • Report incidents, safety concerns, or unusual findings per policy
Care Coordination & Teamwork
  • Communicate effectively with nursing staff, leadership, and interdisciplinary team members
  • Support a safe and respectful environment for residents, families, and coworkers
  • Participate in shift-to-shift communication to ensure continuity of care
Environment & Support Services
  • Provide light housekeeping and resident support services as needed
  • Assist with meal-related needs, hydration, and nourishment monitoring (as applicable overnight)
  • Follow all procedures related to admissions, discharges, and transfers
