There, you'll serve as part of an amazing nursing team providing patient-centered care and services to residents in our assisted living, memory support, and/or skilled nursing neighborhoods.
In this role you will be responsible for assessing, planning and providing quality nursing care to residents according to Federal, State and Mather standards of nursing care. Oversee and supervise CNAs in administering quality care and services to residents. Creates meaningful moments in the lives of residents, visitors, and staff by demonstrating Nextraordinary behaviors.
This part-time, hourly role requires working 12-hour shifts and the ability to work a flexible schedule, including weekdays, evenings, weekends, and holidays.
ESSENTIAL FUNCTIONS
- Follows policies and practices to ensure consistency with current standards promulgated by and appropriate local, state, and federal regulatory bodies and within limits of the respective State Nurse Practice Act.
- Participates in a quality assessment/performance improvement program.
- Is competent in acting in emergency situations based on Safety Manual, Internal and External Emergency Preparedness and other policies.
- Complies with Mather Code of Conduct, Policies, Compliance Program, and all applicable laws and regulations in performance of one's job duties; reports all known and suspected violations to his/her supervisor or manager, Mather Compliance Officer, a Human Resources representative, or the confidential employee helpline.
- Identifies areas for improvement, proposes possible solutions, and participates in data collection and process changes as part of the Quality Assurance Process Improvement process.
- Collects data about each resident's physical, psychosocial, environmental, spiritual, cultural, educational and discharge planning needs, as appropriate.
- Participates in the establishment of desired outcomes with resident, family, and interdisciplinary team.
- Provides input for the development of a care plan/service plan.
- Implements interventions established in the care plan/service plan.
- Delegates tasks based on resident need and according to knowledge and skill of the designated caregiver.
- Performs personal care for residents, as needed, i.e. toileting, bathing, etc.
- Documents nursing care per documentation policy and standards.
- Applies the concepts of the Mather Wellness Model and the 3 A's in the care of residents, Achievement, Autonomy, and Affiliation.
- Uses a customer -focused approach to problem solving philosophy.
- Maintains a courteous and professional manner through interactions with others.
- Communicates with residents and families in a timely manner.
- Uses discretion in handling confidential information, incorporating all Federal, State and local privacy and confidentiality requirements.
- Provides input for all relevant human resource functions including but not limited to: hiring, retention, disciplinary actions, and performance evaluations.
- Responsible for upholding resident rights.
- Participates in resident family education and counseling.
