As we continue to grow, we are seeking compassionate, dependable, and experienced Hospice Registered Nurses (RNs) to join our team. Full-time and part-time opportunities are available.

As a Hospice Registered Nurse, you will provide skilled and compassionate nursing care to patients with life-limiting illnesses in the comfort of their homes.

You will assess patient needs, manage pain and symptoms, coordinate care, educate and support patients and caregivers, and help ensure that every patient receives individualized care focused on comfort, dignity, and quality of life.

You will work closely with our Hospice Clinical Director and interdisciplinary team, including physicians, nurses, social workers, chaplains, hospice aides, and administrative staff, to provide coordinated and patient-centered care.

We believe hospice nurses should be able to spend their time caring for patients—not getting overwhelmed by administrative work.

Our experienced clinical and back-office teams provide strong day-to-day support so our field nurses can work efficiently and independently. With company-provided technology, remote charting capabilities, and a responsive office team, you will have the resources you need to succeed while maintaining flexibility and independence in the field.

Job Types: Full-Time & Part-Time

• Provide skilled nursing care in accordance with each patient's individualized hospice plan of care.
• Perform comprehensive nursing assessments and ongoing evaluations of patients' physical, emotional, and comfort needs.
• Provide effective pain and symptom management to promote comfort and quality of life.
• Administer medications and treatments and evaluate patient response and effectiveness.
• Monitor changes in patient condition and communicate promptly with the Hospice Clinical Director, physicians, and interdisciplinary team.
• Educate patients, families, and caregivers regarding medications, symptom management, disease progression, safety, and end-of-life care.
• Provide emotional support and guidance to patients and families throughout the hospice journey.
• Coordinate care with physicians, hospice aides, social workers, chaplains, facilities, pharmacies, and other members of the care team.
• Participate in interdisciplinary team meetings, care planning, and coordination of services.
• Complete accurate and timely clinical documentation using Kinnser/WellSky on company-provided devices.
• Maintain compliance with hospice regulations, company policies, clinical standards, and safety requirements.
• Serve as a trusted clinical resource and advocate for patients and their families.

• Current and unrestricted Registered Nurse (RN) license in the State of California.
• Minimum of 1 year of nursing experience.
• Hospice or Home Health experience is highly preferred.
• Strong clinical assessment, critical-thinking, communication, and organizational skills.
• Comfortable managing patients independently in the field while collaborating closely with an interdisciplinary team.
• Compassionate, dependable, and patient-centered approach to end-of-life care.
• Ability to communicate effectively and compassionately with patients, families, caregivers, and healthcare professionals.
• Valid California driver's license and reliable transportation.
• Ability to travel throughout the assigned service area.
• Comfortable using electronic medical records and mobile technology for clinical documentation.
