About the Role:
Divine Guardian LLC is seeking a compassionate and dedicated Hospice RN to join our caring team in Tucker, GA. Responsibilities:
  • Provide skilled nursing assessments and direct patient care to hospice patients in home or facility settings
  • Develop, implement, and update individualized patient care plans in coordination with the interdisciplinary team
  • Manage pain and symptom control to maximize patient comfort and quality of life
  • Educate and support patients and family members on disease progression, medications, and end-of-life care
  • Coordinate with physicians, social workers, chaplains, and aides to ensure holistic patient care
  • Accurately document patient assessments, care plans, and clinical notes in compliance with hospice regulations

Requirements:
  • Active RN license in the state of Georgia (required)
  • Minimum 1-2 years of nursing experience; hospice, palliative care, or home health experience preferred
