Job description

Position Summary: The incumbent conducts individual assessments with residents of public housing; identifies local resources to assist them in aging in place and increasing quality of life and linking them to said providers for service provision. Follows-up to ensure grant goals and objectives are met according to grant guidelines. Compiles and maintains program coordinating committee of service providers to evaluate service provision and makes recommendations for additional service providers as the program progresses.

ESSENTIAL FUNCTIONS:

  1. Ensures enforcement of HUD regulations and guidelines relative to ROSS program operation, administration and compliance.
  2. Serves as liaison and coordinating agent between the Springfield Housing Authority and public/private agencies that provide community services, commensurate with the needs and interest of Authority residents.
  3. Coordinates Program Coordinating Committee for ROSS SC Program.
  4. Prepares proposals for services needed by residents, monitors delivery of service providers and takes appropriate action(s); maintains a working relationship with service providers; and networks for additional resources in the community.
  5. Maintains, updates, and revises procedures and policies as necessary, to remain current with HUD regulations and/or when current practices are hindering the success of the Service Coordinator program.
  6. Processes applications for participants enrolled in Service Coordinator program; determines eligibility, conducts client intake, provides supportive counseling, activity and resource referrals, goal development and planning, ongoing case management; monitors progress/activities; revises goals and objectives as needed.
  7. Prepares, coordinates, and submits monthly and quarterly housing reports for the supervisor/Asset Manager, board meetings, and HUD upon request, in a timely manner.
  8. Establishes long- and short-term division priorities.
  9. Maintains records for reporting.
  10. Attends executive and divisional staff meetings.
  11. Researches information for tenant's individual service plans.
  12. Organizes and inputs data into an e-logic model.
  13. Ensures compliance with all applicable policies and procedures of the Springfield Housing Authority.
  14. Maintains regular and reliable attendance.
  15. Contributes to team efforts by performing other duties and responsibilities as required or assigned.
