ConnextCare is expanding and seeking a passionate, experienced Nurse Practitioner to join and grow our newest office in the Hannibal School District. This unique opportunity places you at the heart of a School-Based Health Center, where you'll provide vital care to students and help shape healthier futures every day.
Whether you're looking for a 10, 11, or 12-month schedule, this role offers flexibility, purpose, and the chance to build meaningful relationships with the community you serve.
As a Nurse Practitioner at ConnextCare, you will:
  • Provide comprehensive patient care, including diagnosis, treatment, and referrals
  • Deliver care within your scope of practice, collaborating with physicians as needed
  • Maintain accurate, compliant medical records in accordance with all regulations
  • Participate in quality improvement initiatives, peer review, and continuing education
  • Support development and monitoring of healthcare goals and quality metrics
  • Engage in community outreach, helping to promote the role of the Nurse Practitioner
  • Collaborate with leadership and medical staff to improve care delivery and efficiency
  • Contribute to Patient-Centered Medical Home (PCMH) initiatives

Requirements
Knowledge/ Education Requirements:
A. Valid/active NYS RN and NP licensure
B. Masters degree in a Nursing
C. Experience in family practice preferred, but not required
D. Computer proficiency in electronic health records, health information technology and health informatics required
E. Promote the organization's code of conduct, safety rules, and adherence to all company policies
F. Carry out job responsibilities in an ethical, effective, comprehensive and professional manner
G. Cooperate, work, and communicate with staff, supervisors, management, and patients in a professional manner
H. Display compassion and work well in emergent and stressful situations
I. Demonstrate and enforce standards for safe, quality patient care, setting high standards by example
J. Must be keenly aware of the importance of confidentiality in all aspects of the position
