Biotech Advance Medical Center is seeking a compassionate and dedicated Full-Time Community-Focused Nurse Practitioner, based in Hialeah, Florida. The position is onsite, primarily focusing on home care services, allowing for direct engagement with patients in their environments. You can expect to work approximately 40 hours per week, typically from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m. Participation in community outreach, including health fairs and weekend educational events, is integral to the role and essential for fostering community relationships.

• Perform thorough health assessments and screenings for individuals and families, utilizing electronic health record systems to maintain detailed documentation.
• Diagnose and develop treatment plans for acute and chronic medical conditions, implementing evidence-based practices to guide patient care.
• Collaborate with local community organizations to design and launch health education initiatives that address prevalent health issues, such as diabetes management and preventive screenings.
• Provide culturally competent care, advocating for patients' rights and ensuring that their healthcare needs are met, regardless of their backgrounds.
• Facilitate referrals to specialists and follow up on patient care plans to guarantee continuity of care, utilizing a team-based approach that involves other health professionals.
• Engage in continuous learning to stay updated with the latest community health trends, regulatory requirements, and best practices.

This role primarily supports diverse, underserved populations within the Hialeah area, addressing various health disparities through community outreach and education. You will focus on empowering individuals by providing them with the resources and knowledge needed to manage their health effectively. • Valid certification as a Nurse Practitioner in the state of Florida.
• Current and active state nursing license.
• A Master’s degree in nursing from an accredited institution.
• Proven expertise in primary care or community health, demonstrating strong assessment and problem-solving skills.

• At least 3-5 years of clinical experience in community health or public health settings is preferred.
• Experience in conducting health education workshops or participating in outreach initiatives.
• Proficient in utilizing electronic health records and health informatics tools to enhance patient care and documentation.
