The role
The Care Navigator Assistant provides essential virtual and administrative assistance to the Care Navigation team. This role is responsible for eliminating administrative bottlenecks, managing patient panels, and tracking daily/weekly engagement goals to ensure no patient falls through the cracks. The ideal candidate is highly organized, data-driven, and capable of seamlessly managing medical records, partner trackers, and real-time clinical support requests.

Key Responsibilities:
Panel Management & Goal Tracking
  • Daily List Management: Actively monitor and clear daily patient list, routing patients to assigned Care Navigators to ensure 100% panel outreach.
  • Metric Tracking: Help monitor team progress toward check-in goals (e.g., tracking progress toward our 1,200-check milestones) and monitor tracker spreadsheets with partner organizations.
  • Time Compliance Monitoring: Review documentation logs to ensure every patient interaction meets the mandatory minimum of 20 minutes of billable time.
Records, Faxes, & Clinical Administration
  • Inbox & Records Management: Manage the digital fax inbox, process incoming medical records requests, and perform routine "chart housekeeping" within the EHR.
  • Insurance Coordination: Handle eligibility and benefits verification for patients on the panel.
  • Care Plan Coordination: Send out care plans, visit notes, and formal referrals as requested by Care Navigators (CNs).
  • Specialized Tracking: Request neuroimaging reviews immediately upon receipt of patient images, and notify providers/CNs when home safety assessments are completed
Patient Outreach & Support
  • Reminders & Confirmations: Send out appointment confirmations and reminders to maximize patient show rates.
  • Resource Coordination: Under the direction of Care Navigators, research and locate local community resources for patients as needed.
  • Just-in-Time Support: Provide agile, in-the-moment administrative support for Care Navigators to keep daily workflows running smoothly.
Qualifications & Skills
  • Experience: 1-3 years of administrative or virtual assistant experience, ideally within a care management, neurology, brain health or busy medical clinic setting.
  • Technical Savvy: Proficient with electronic health records (EHR), digital fax platforms, and tracking spreadsheets (Excel/Google Sheets).
  • Metric-Driven Mindset: Comfortable working in an environment with clear numerical targets (outreach goals, tracking metrics, minimum billable minutes).
  • Communication: Exceptional written and verbal communication skills; professional and empathetic when sending patient reminders and coordinating with partner organizations.
