Care Coordinator Navigator

A Care Coordinator Navigator assists individuals in navigating the healthcare system, coordinating care services, and advocating for the needs of patients. They work closely with healthcare providers, patients, and families to ensure that the appropriate care is provided and that patients receive the support they need. This role may involve scheduling appointments, providing information on available resources, coordinating referrals to specialists, and assisting with insurance and billing issues. Care Coordinator Navigators play a vital role in helping patients access and navigate the complex healthcare system by supporting teams with non-clinical patient interactions with the goal to reduce barriers to care and to increase patient satisfaction.

What We're Looking For
