Clinical Coordinator / Navigator

Summary:

The Clinical Coordinator/Navigator serves as a key liaison between patients, providers, staffing services, and internal teams to ensure a positive patient experience and seamless coordination of care. This position is primarily office-based and responsible for administrative and patient support functions, including coordinating documentation, communicating with staffing partners, conducting patient satisfaction surveys, tracking service outcomes, and assisting with care navigation activities.

Key Responsibilities:
  • Coordinate communication between patients, providers, staffing services, and internal departments.
  • Send physician orders, chart notes, care plans, and other documentation to staffing agencies and care partners.
  • Monitor documentation requirements and follow up on missing records.
  • Conduct patient satisfaction surveys and document results.
  • Track patient feedback and identify service improvement opportunities.
  • Assist patients and families with questions regarding services and care coordination.
  • Maintain accurate patient records within EMR systems.
  • Follow up with patients to ensure services have been initiated and needs are being met.
  • Assist with patient onboarding, intake, and coordination activities.
  • Generate reports related to patient satisfaction, referral activity, and service outcomes.
  • Ensure compliance with HIPAA and company policies.
  • Perform other duties as assigned.
Requirements:

Qualifications:

  • High school diploma or equivalent required; Associate's or Bachelor's degree preferred.
  • Minimum two years of healthcare coordination, patient navigation, customer service, or medical office experience.
  • Strong organizational and administrative skills.
  • Excellent verbal and written communication skills.
  • Experience with EMR systems and Microsoft Office.
  • Ability to manage multiple priorities in a fast-paced environment.
  • Strong customer service and patient-focused approach.
