POPULATION SERVED: Navigate provides intensive, community-based services to individuals with severe mental illness. Individuals served by Navigate often have complex mental health needs, including acute psychosis, mood dysregulation, histories of complex trauma, and co-occurring substance use disorders. The primary function of Navigate is to help clients experiencing a first episode of psychosis or bipolar disorder find their individual pathways to recovery by improving their social, leisure, and role functioning, as well as increasing overall well-being, all while encouraging family education and support along the way.
POSITION SPECIFIC DUTIES & RESPONSIBILITIES:
  • Work with clients to identify their personal preferences regarding work and school goals, including career interests, and then provides the necessary supports to help the person achieve those goals.
  • Attend and participate in daily organizational meetings and weekly treatment team meetings with multi-disciplinary team, utilizing a team approach in working with Navigate clients.
  • In the initial assessment phase, complete an Education and Career Inventory with the clients to develop a shared understanding of the individual, their employment or school history, and their preferences with respect to school, types of work, and career backgrounds.
  • The job search or school enrollment phase is aimed at providing practical assistance to the client in finding the type of job or school program that they are interested in participating in.
  • The final phase involves providing follow-along supports that are aimed at giving clients practical assistance to facilitate good performance, prevent crises from arising, and promote success at work or school.
  • Be available to respond to crises in the community, utilizing agency protocols for crisis diversion. May include completion of Third-Party Affidavits and coordination with law enforcement to ensure individual is safe.
  • Utilize agency issued cell phone during work hours. Staff are expected to be responsive to cell calls and agency approved electronic communication, when scheduled to work.
  • Have reliable transportation and be willing to utilize personal vehicle for community-based work, which may include transporting clients when appropriate.
  • Demonstrate comfort and flexibility while working with clients in difficult life circumstances, which may include individuals, who are homeless or living in substandard conditions.
  • Must have a strong commitment to the rights and ability of each person with a severe and persistent mental illness. Skills to establish supportive, trusting relationships and respect for client preferences in treatment are essential.

QUALIFICATIONS: Case Managers for Navigate must have a Bachelor's degree in Social Work, Psychology or a related social services major, which qualifies for Case Management II certification with the Oklahoma Department of Mental Health. Candidates must have CM-II certification or obtain CM-II certification within 60 days of employment. Applicants with a previous background working in community mental health, inpatient psychiatric facilities, criminal justice, home health, or similar settings are preferred. Strong candidates with a passion for helping individuals with severe mental illness will also be considered. Must have a valid driver's license and satisfactory driving record; use of personal automobile will be required for travel to locations outside of primary office.
CCBHC Model of Care
Family & Children's Services works to heal hurting and abused children, strengthen families, and provide hope and a path to recovery for those battling mental illness and addiction. You will be working in an innovative and dynamic environment using a new transformative model of care Certified Community Behavioral Health Clinic (CCBHC). This model is characterized by innovative, team-based approach and whole person care for adults and children with a range of complex mental health and substance use challenges.
Utilizing multi-disciplinary teams, staff work collaboratively to provide a coordinated effort to enhance client recovery. Robust CCBHC services often result in better client outcomes and quality of care due to:
  1. Increased access to care and crisis services
  2. Expanded traditional community mental health and substance use services.
  3. Added Care Coordination and physical health screening for mental health clients
  4. Greater access to Social Services for clients' economic and social needs
  5. Increased specialized services for veterans, those most in need, and those impacted by the opioid crisis.
