We're looking for a compassionate, relationship-driven Client Care Manager (CCM) to join our growing team. This role is perfect for someone who enjoys helping families navigate care decisions, building community relationships, ensuring exceptional client experiences, and making a meaningful impact every day.

What You'll DoClient & Family Support
  • Answer inquiries with empathy and professionalism.

  • Educate families about Right at Home services and available community resources.

  • Conduct in-home assessments and develop individualized care plans.

  • Coordinate care starts and provide ongoing support to clients and families.

  • Ensure clients receive safe, meaningful, and high-quality care.

Quality Assurance
  • Conduct announced and unannounced supervisory visits.

  • Monitor client satisfaction and address concerns promptly.

  • Conduct caregiver/client introductions and training as needed.

  • Provide direct client care when necessary.

  • Encourage client feedback and Google reviews.

Community & Relationship Building
  • Build and maintain relationships with hospitals, clinics, assisted living communities, skilled nursing facilities, social workers, case managers, and other healthcare professionals.

  • Visit referral partners regularly and represent Right at Home at community events, networking opportunities, and health fairs.

Team Support
  • Collaborate with Recruiting, Scheduling, and Operations teams.

  • Share caregiver performance feedback and client observations.

  • Assist with phone coverage and scheduling needs as required.

Qualifications
  • High School Diploma or GED required.

  • Associate Degree in Social Work, Human Services, Healthcare, or related field preferred.

  • Active Nebraska CNA License required.

  • Strong communication, relationship-building, and organizational skills.

  • Knowledge of common medical terminology.

  • Ability to work independently and exercise sound judgment.

  • Valid driver's license, auto insurance, and reliable transportation.
