Job Description: Reporting to the Facility Manager, the Facility Care Coordinator is responsible for ensuring a high standard of care and service is provided to patients and residents at Wolseley Memorial Integrated Care Center (WMICC). WMICC is comprised of 78 beds in Long Term Care, 12 Bed Acute Care and OPD services. As an experienced practitioner, the facility care coordinator acts in a leadership role to provide specialized clinical expertise, coordination of client care, and support and teaching to the client care teams (including clients and their families) within a collaborative environment.
Human Resources Exemption: No
Experience

  • Minimum of 3 years clinical nursing experience
  • Recent acute and long term care experience required.
  • Supervisory experience is an asset
  • Minimum of 3 years clinical nursing experience
  • Experience in acute care

Education

  • Bachelor of Science in Nursing (BScN)

Other Education and Training

  • Basic Cardiac Life Support Certificate (BCLS)
  • Experience in a clinical leadership role an asset
