JOB SUMMARY:

Cornerstone is committed to total satisfaction through delivery of services, quality of our life skills training and support, and the continual improvement of our quality management system within our Youth Residential Services. The Qualified Intellectual Disabilities Professional (QIDP) is responsible for the partnering with the Program Director, Deputy Program Director, Intensive Behavioral Intervention (IBI) Specialist and the direct care staff at the assigned service site(s). Each child’s active treatment program must be integrated, coordinated and monitored by a Qualified Intellectual Disabilities Professional (QIDP). Generally, the Qualified Intellectual Disabilities Professional (QIDP) ensures:

• service design and delivery which provides each child with an appropriate active treatment program;

• that any discrepancies or conflicts between programmatic, medical, dietary and vocational aspects of the child’s assessment and program are resolved; and

• a follow-up to recommendations for services, equipment or programs.

This Position will require working direct care shifts and/or preforming direct care work.

The tasks are highly complex and require intellectual disability management skills, negotiation and communication skills, organizational abilities, advocacy skills, and overall business sense.

ESSENTIAL DUTIES AND RESPONSIBILITIES

1. The Qualified Intellectual Disabilities Professional (QIDP) shall complete an initial treatment plan for each child, which must be completed within 7 days of admission. The treatment plan must address the child’s immediate needs and meet the requirements of 465 IAC 2. Subsequently, within 30 days of admission the Treatment Plan must be updated to meet the requirements of an Individual Program Plan (IPP).
2. The Qualified Intellectual Disabilities Professional (QIDP) shall complete a developmentally based comprehensive functional assessment within 30 days of a child’s admission. The comprehensive functional assessment must yield data that is accurate, reflects the current status and needs of the individual, and can serve as a functional basis for an Individualized Program Plan (IPP) to be developed.
3. The comprehensive functional assessment developed by the Qualified Intellectual Disabilities Professional (QIDP) must address the child’s:

• Physical development and health;

• Nutritional status;

• Sensorimotor development;

• Affective (emotional) development;

• Speech and language (communication) development;

• Auditory functioning;

• Cognitive development;

• Vocational development;

• Social Development; and

• Adaptive behaviors or independent living skills.

1. The functional assessment developed by the Qualified Intellectual Disabilities Professional (QIDP) must also identify the child’s:

• Specific developmental strengths, including child preferences;

• Specific functional and adaptive social skills the child needs to acquire;

• Presenting disabilities, and when possible their causes;

• Need for services; and

• Specific developmental and behavioral management needs.

1. The Individual Program Plan (IPP) developed by the Qualified Intellectual Disabilities Professional (QIDP) must include opportunities for the child’s choice and self-management and must identify:

• the discrete, measurable, criteria based objectives the child is to achieve;

• and the specific individualized program of specialized and generic strategies, supports, and

techniques to be employed.

1. The Individual Program Plan (IPP) developed by the Qualified Intellectual Disabilities Professional (QIDP) must be directed toward the acquisition of the behaviors necessary for the child to function with as much self-determination and independence as possible.
2. The Individual Program Plan (IPP) developed by the Qualified Intellectual Disabilities Professional (QIDP) must state the specific objectives necessary to meet the client's needs, as identified by the comprehensive functional assessment and the planned sequence for dealing with those objectives. These objectives must:

• Be stated separately, in terms of a single behavioral outcome
• Be assigned projected completion dates
• Be expressed in behavioral terms that provide measurable indices of performance
• Be organized to reflect a developmental progression appropriate to the child; and
• Be assigned priorities.

1. Until it has been demonstrated that the child is developmentally incapable of acquiring these skills, the Individual Program Plan (IPP) developed by the Qualified Intellectual Disabilities Professional (QIDP) must also include, for those children who lack them, training in skills essential for privacy and independence including, but not limited to:

• toilet training

• personal hygiene

• dental hygiene

• self-feeding

• bathing,

• dressing

• grooming

• communication of basic needs

1. Prior to implementation, each objective on the Individual Program Plan (IPP) developed by the Qualified Intellectual Disabilities Professional (QIDP) shall have a written training program designed to facilitate the child’s achievement of each objective. Objectives may be implemented based on priority. Each written training program must specify:

• The methods to be used;

• The schedule for use of the method;

• The person(s) responsible for implementing the program;

• The type of data and frequency of data collection necessary to be able to assess progress toward the desired objectives;

• The inappropriate client behavior(s), if applicable;

• Provision for the appropriate expression of behavior and the replacement of inappropriate behavior, if applicable, with behavior that is adaptive or appropriate.

1. Except for those facets of the Individual Program Plan that must be implemented only by licensed personnel, the Qualified Intellectual Disabilities Professional (QIDP) shall ensure that each client's individual program plan must be implemented by all staff who work with the client, including professional, paraprofessional and nonprofessional staff.
2. The individual program plan must be periodically reviewed, at least by the Qualified Intellectual Disabilities Professional (QIDP), and revised as necessary in situations in which the client has successfully completed an objective or objectives identified in the individual program plan, is regressing or losing skills already gained; is failing to progress toward identified objectives after reasonable efforts have been made or is being considered for training towards new objectives.
3. The Qualified Intellectual Disabilities Professional (QIDP) shall be a part of the interdisciplinary team that reviews the comprehensive functional assessment of each child at least annually. The comprehensive functional assessment must be reviewed for its relevancy and updated, as needed.
4. The individual program plan (IPP) developed by the Qualified Intellectual Disabilities Professional (QIDP) must also identify mechanical supports, if needed, to achieve proper body position, balance, or alignment. The individual program plan (IPP) must also include generalization of skill training to the community when safe and appropriate.
5. The Qualified Intellectual Disabilities Professional (QIDP) follows established clinical counseling procedures and related policies throughout all areas of Cornerstone.
6. The Qualified Intellectual Disabilities Professional (QIDP) adheres to all policies and procedures as defined in the Employee Handbook and Cornerstone Operational Policies and Procedures Manual.

REQUIREMENTS

EDUCATION AND EXPERIENCE

1. A Qualified Intellectual Disabilities Professional (QIDP) is a person who has specialized training or one (1) year of experience in treating youth with IDD, and is one (1) of the following:

• A licensed psychologist.
• A licensed doctor of medicine or osteopathy.
• An educator with a degree in education from an accredited program.
• A social worker with a bachelor's or master's degree in social work from an accredited program.
• A person with a bachelor's or master's degree in a human services field other than social work and at least three (3) years of social work experience under the supervision of a qualified social worker or at least one year of experience working directly with persons with intellectual disabilities or other developmental disabilities.
• An occupational therapist who is a graduate of an occupational therapy curriculum accredited jointly by the council on medical education of the American Medical Association and the American Occupational Therapy Association; is eligible for certification by the American Occupational Therapy Association under its requirements in effect on September 29, 1978; or has two (2) years of appropriate experience as an occupational therapist and has achieved a satisfactory grade on the approved proficiency examination, except that such determinations of proficiency shall not apply with respect to persons initially licensed by the state or seeking initial qualifications as an occupational therapist after December 31, 1977.
• A speech pathologist or audiologist licensed pursuant to IC 25-35.6-3.
• A registered nurse licensed pursuant to IC 25-23-1-11.
• A therapeutic recreation specialist who is a graduate of an accredited program.
• A rehabilitative counselor who is certified by the Committee of Rehabilitation Counselor Certification.
• A physical therapist that is licensed pursuant to IC 25-27.
• A person with a master’s degree in psychology from an accredited program. Master’s degree in applied behavioral analysis; social work; psychology; counseling; or a related human services area of study approved by DCS.

1. Minimum age of 21 per licensure requirements.
2. Valid Indiana driver’s license and driving record that meets eligibility requirements of Cornerstone’s insurance carrier (for all positions requiring driving).
3. Must meet and maintain background screening requirements of program(s) assigned to.
4. No felony convictions or other criminal offenses involving a dependent population.
5. Successful completion of pre-employment physical.
6. Ability to work flexible hours and locations and cover direct care shifts as necessary.
7. Experience in managing systems, processes, and people.
8. Valid Mantoux tuberculin test or chest x-ray within thirty days of employment.

BUSINESS AND TECHNICAL SKILLS

1. Ability to apply general understanding and knowledge to carry out instructions in written, verbal or schedule form.
2. Ability to handle and carry out multiple tasks.
3. Basic computer skills with the ability to learn and effectively utilize identified computer software applications to accomplish tasks.
4. Ability to operate general office, household and adaptive equipment.
5. Ability to read, comprehend, interpret and apply information from general procedures, safety rules, instructions, data and reports.
6. Ability to use Basic English grammatical skills to write routine reports and general correspondence.
7. Organizational skills and ability to accurately document data and maintain records.

GENERAL REQUIREMENTS AND WORKING CONDITIONS

1. Must have ability to work a flexible schedule either in-person or remotely, which may include days, evenings, weekends, and/or overnight shifts.
2. Must provide own transportation, have valid driver’s license and driving record must meet insurability requirements.
3. Must take and successfully complete medication administration training, prior to start date.
4. Must maintain and provide documentation of auto insurance.
5. Must have ability to lift/move/transfer individuals or materials weighing up to 75 lbs. periodically.
6. Must have ability to stand, walk, stoop, and bend frequently.
7. Must have manual dexterity sufficient to reach, handle items, work with the fingers, and perceive attributes of objects and materials (such as medication, first aid supplies, with meal preparation and household tasks).
8. Must maintain CPR and First Aid certification and must stay current on all other training as defined by Indiana state guidelines, industry accreditation standards and company guidelines.
9. Must successfully complete background check at the time of hire (prior to delivery of services to youth) and annually thereafter.
10. While performing the duties of this job, the employee is occasionally exposed to moving mechanical parts, toxic and caustic chemicals, and fumes or airborne particles. The noise level in the work environment is usually quiet to moderate.
