How you’ll make an impact in this role
- Secure service authorizations from insurance providers to ensure patients have uninterrupted access to the medical equipment and specialty care they need for recovery.
- Coordinate patient transitions by identifying appropriate recovery facilities and home health agencies, ensuring every patient is moved to a safe environment that honors their personal choice.
- Expedite the discharge process by preparing and delivering essential medical documentation to receiving providers, minimizing wait times and ensuring a seamless hand-off of care.
- Bridge communication gaps between hospital staff, admissions, and insurance payers to keep the care team informed and resolve any coverage hurdles in real time.
- Maintain accurate care records by documenting every step of the patient’s journey to provide a clear, high quality history that supports better long-term health outcomes.
Education:
- High School diploma equivalency OR 1 year of applicable cumulative job specific experience required.
- Note: Required professional licensure/certification can be used in lieu of education or experience, if applicable.
- Healthcare experience.
