Senior LTSS Service Care Manager (RN)
Location: Weslaco, Mercedes, Harlingen, TX & surrounding areas
Duration: 3+ Month Contract (Potential Extension/Conversion)

We are seeking an experienced Registered Nurse (RN) with a background in Case Management or Home Health to support members with complex medical needs. In this role, you'll coordinate long-term care services, develop personalized care plans, collaborate with healthcare providers, and advocate for members to ensure they receive high-quality, cost-effective care.
Key Responsibilities
Assess members with complex medical and long-term care needs.
Develop, implement, and update individualized care plans.
Coordinate care with providers, caregivers, and community resources.
Monitor member progress and adjust care plans as needed.
Educate members and families on healthcare services, benefits, and disease management.
Maintain accurate documentation and ensure regulatory compliance.
Conduct home or community visits when required.
Required Qualifications
Active Registered Nurse (RN) license (State or Compact License).
Graduate of an accredited School of Nursing.
1-2+ years of Case Management or Home Health experience.
Strong computer and telephone communication skills.
Ability to coordinate care for complex or high-acuity patients.
Willingness to travel for member visits as needed.
Preferred Qualifications
Bachelor's degree in Nursing (BSN).
4-6 years of nursing or care management experience.
Experience in Managed Care, LTSS, Medicaid, or Medicare.
Knowledge of community resources and care coordination.
