Shift: Sunday-Thursday, 10:00am-6:00pm

Lifeworks is hiring a IHS Case Manager to work with Individuals with ID/DD. We are a nonprofit human service agency that provides adults with developmental disabilities with the opportunity to live engaged and meaningful lives. We're searching for an enthusiastic individual who will bring their personality, an independent work ethic, and a strong belief in enhancing the lives of those we serve. *Candidates must have related or direct experience working with the community we serve.
Essential Duties:
  • Provide care and assistance to individuals who live independently in the community. Assist individuals with tasks such as banking, grocery shopping, budgeting, paying bills, planning vacations, managing healthcare, and coordinating and participating in a variety of one-on-one and group social activities.
  • Promote optimal health for individuals through routine, preventive and emergency health care. Ensures that residential environments are safe, secure and where individuals are free from harm.
  • Provide education training, counseling, and physical assistance when necessary to achieve ISP goals in the following areas: functional academics, domestic skills health and personal hygiene, social and leisure skills, communication skills, community mobility, self-esteem, money management.
  • Develop, write and present annual ISP's and semi annual assessments in consultation with other residential staff and Director.
  • Provides transportation to individuals as necessary.
  • Communicates with persons significant to the individual served (i.e. family, friends, guardians, funding sources, advocates, volunteers, employers) to facilitate consistency of programming and support.
  • Performs case management duties including, maintenance of individual confidential files, ensuring cases are reviewed, documentation of activities and training.
  • Attends regular meetings as necessary.
  • Other duties and/or assignments to work/program locations based on the needs of the company.
Requirements:
  • Experience working with Individuals with ID/DD
  • Bachelor's degree from four-year college or university; related experience preferred.
  • Must be able to pass Fingerprint background check, CORI, DPPC and Driving background checks
  • Valid Driver's License required
  • Excellent verbal and written communication skills
