Legacy Community Health has been serving communities in the Gulf Coast Region for over 40 years.
In partnership with AmeriCorps, we are offering an opportunity to earn $816 bi-weekly stipend this Fall as:

  • AmeriCorps Member - Pharmacy Advocate
  • AmeriCorps Member - Prevention Coordinator
  • AmeriCorps Member - Maternal and Child Health Program Specialist
  • AmeriCorps Member - Connect Community Health Worker

• Work from Home (If applicable)What you can expect to be doing:
As an AmeriCorps Member you will be placed based on organizational need and will help reduce barriers to healthcare by supporting patients, improving health literacy, and connecting families to essential services. Responsibilities:
  • Connect community members to Legacy’s primary care, preventive services, and eligibility programs.
  • Educate the community about Legacy’s full range of healthcare services.
  • Collaborate with community partners to identify additional service opportunities.
  • Perform other service-related tasks within AmeriCorps guidelines.
  • Provide translation or interpretation services when appropriate.
  • Wear the AmeriCorps logo daily as a program representative.
  • May be required or requested to support disaster response and recovery efforts at the local, state, or federal level.
Patient Information Coordination
Members serving in this track will:
  • Help patients navigate Legacy’s healthcare system and overcome access barriers.
  • Greet and assist patients, families, and visitors with information, directions, and wayfinding support.
  • Escort patients to care areas and answer questions to ensure a welcoming, efficient clinic experience.
  • Support coordination of healthcare visits and patient flow.
Pharmacy Advocacy
Members serving in this track will:
  • Help patients access affordable medications and understand available pharmacy services.
  • Provide technical support for pharmacy applications, refill tools, and medication delivery services.
  • Educate patients on medication adherence, self-monitoring devices, and preventive health topics.
  • Assist with medication assistance programs, insurance navigation, and pharmacy workflow improvements.
  • Collaborate with pharmacy staff and participate in quality improvement and educational activities.
Maternal and Child Health Specialization
Members serving in this track will:
  • Serve as Legacy's Peripartum-Pediatric Navigator who makes and keep prenatal and post-partum care appointments for patients
  • Support Legacy's prenatal education program completing patient scheduling, baby care item distribution, and monthly reporting
  • Support Legacy's car seat safety program by completing patient scheduling, car seat inventory, distribution, and monthly reporting
Community Health Worker
Members serving in this track will:
  • Conduct community outreach, health education, and preventive screenings to improve healthcare access and promote wellness among diverse and underserved populations throughout Houston.
  • Assist individuals and families with healthcare navigation, appointment scheduling, insurance enrollment, and referrals to medical, social, and community support services.
  • Build and maintain partnerships with community organizations, hosted outreach events, and represented Legacy Community Health to expand awareness of primary and preventive healthcare programs.
  • Provide culturally responsive case management, follow-up support, and health advocacy while connecting patients to chronic disease prevention, early childhood development, and ongoing healthcare resources.
Outreach and Enrollment Specialization
Members serving in this track will:
  • Provide education to community members on available healthcare programs and services at Legacy Community Health.
  • Form relationships with local community organizations to bridge gaps in care for Legacy patients and foster stronger connections between Legacy and the communities served.
  • Assist patients in addressing financial barriers to receiving primary and preventative healthcare services
Prevention Coordination
Members serving in this track will:
  • Provide phone contact, to motivate, and encourage patients to complete their cancer screenings.
  • Provide reminders to patients regarding appointments scheduled.
  • Acts as liaison between patient, Legacy provider, and all other community providers.
Literacy Coordination (Little Reader’s Program)
Members serving in this track will:
  • Provide literacy resources to pediatric patients and families to support healthy development.
  • Read to children in clinic Little Reader Corners and distribute literacy materials.
  • Participate in community outreach such as book drives, story times, and health fairs.
  • Connect families to community resources that address literacy-related social determinants of health.
