Cambrian Homecare, LLC has been providing compassionate, flexible, and personalized in-home care services since 1996, helping seniors and individuals with developmental disabilities live independently in the comfort of their homes.

We are seeking a compassionate, capable, and dependable Full-Time Intake Coordinator to join our team at our corporate office in Long Beach. The Intake Coordinator plays an essential role in converting new referrals, guiding clients and families through the onboarding process, and maintaining strong relationships with referral sources. This position helps ensure each new client receives a smooth, timely, and supportive intake experience from the first point of contact through the transition to care.
Schedule: Tuesday - Saturday 8:30AM - 5:30PM
Responsibilities
Referral Conversion
  • Manage new referrals in accordance with department standards and company guidelines.
  • Conduct timely outreach to prospective clients, families, and referral sources.
  • Maintain accurate and up-to-date client status information.
  • Partner with referral sources to obtain updated client information and support successful conversion.
Client Onboarding
  • Educate clients and families on available services to support a successful transition to care.
  • Document intake activities accurately and as directed.
  • Communicate relevant client information to the staffing team to support an appropriate caregiver match.
  • Help ensure clients and families feel informed, supported, and confident throughout the onboarding process.
Relationship Building
  • Respond to referral emails and inquiries in a timely and professional manner.
  • Provide referral sources with timely updates regarding client and family outreach.
  • Maintain referral source portals with current and accurate information.
  • Build and maintain positive working relationships with referral sources.

  • Associate's degree or equivalent combination of relevant education and experience.
  • Strong verbal and written communication skills.
  • Ability to work effectively in a fast-paced environment.
  • Comfortable managing a high volume of calls and client interactions.
  • Strong attention to detail and organizational skills.
  • Fluency in both English and Spanish.
Job Requirements
  • Must be at least 18 years of age.
  • US Work Authorization.
  • Ability to pass a background check and reference check.
