Sertoma is currently seeking a Full-Time Case Manager for our Employment and Community First (ECF) and Residential programs. The shift is full-time, and this is a non-exempt / hourly position with a payrate of $16.50 an hour.

ECF Case Manager Job Summary:

Responsible for implementing/monitoring the Person-Centered Support Plans in the Employment and Community First (ECF) and Residential programs.


ECF Case Manager Relationships:

Reports To: ECF Program Director

Supervises: No supervisory responsibilities

Works With: ECF Program Director, ECF Job Developer, Chief Operating Officer, Senior Director of Residential Services, Residential Coordinators, Clinic Staff, Senior Director of Day Services, Community Coordinator, ECF Program Staff, Day Program Staff, Residential Staff, other Case Managers, Business Office Staff, Quality Assurance Coordinator, Billing Specialist, and Recreation Therapists.

External Relationships: Support Coordinators, Family Members/Conservators/POAs, TennCare, Department of Intellectual and Developmental Disabilities, Therapists, Dieticians, and all other applicable persons.

Requirements:

ECF Case Manager Qualifications / Employment Requirements:


Education: Bachelor’s degree preferred in Social Work, Sociology, Psychology, Special Education, or related field. Or a minimum of 2 years’ experience working with people with disabilities.

Work Experience: One year experience working with persons with special abilities required. Have ACRE training and or be willing to obtain ACRE certification.

License or Registration Requirements: Valid TN driver's license and a good driving record.

Physical Requirements: Ability to sit and/ or stand for long periods of time; may also require some heavy lifting (i.e., toileting clients).


ECF Case Manager Responsibilities:


1. Will thoroughly learn and enforce all Sertoma, Inc. personnel ‘Policies and Procedures’ in an unbiased and professional manner.

2. Will thoroughly learn and enforce all standards as set forth by Licensure, Quality Assurance, Medicaid Waiver, and Sertoma. Will serve as officer for contract compliance in all areas.

3. Will be responsible for completing and monitoring all paperwork requirements of the assigned caseload in a timely manner. This includes but is not limited to the Person-Centered Support Plans, Implementation Plans, Behavior Support Plans, Staff Instructions, Monthly Service Reviews, and various assessments.

4. Assist ECF Program Director in coordinating meetings when accepting new ECF referrals to Sertoma.

5. Coordinate, facilitate, document, and participate in all meetings pertaining to your assigned caseload, and other meetings as may be required.

6. Will train all pertinent staff on individual plans, documentation and required needs of your caseload.

7. Will assist in the communication between ECF Program Director, ECF Direct Support Staff, Residential Coordinators, Support Coordinators, and other Sertoma staff.

8. Will conduct unannounced visits as directed by the Quality Assurance Coordinator.

9. Will have or obtain the necessary training requirements to provide job development services. (ACRE Training)

10. Will provide direct support to members during job development services or on an as needed basis as directed by your supervisor.


ECF Case Manager General Expectations of Performance:


For the Center to have continued success and for the individuals served by Sertoma Center to have a consistently increase in the quality of their lives, there are areas of performance that all staff will always adhere to:


1. You will recognize that your efforts are part of a team function of which all Sertoma Center staff are members. This will be accomplished through showing consistent respect of co-workers, recognition of the diversity that is present in the workplace and practicing an unbiased disposition of Center business.

2. You will contribute to the mission and principles of the Agency by continually being dedicated to serving individuals with disabilities. This will be achieved by remaining alert to new concepts which could improve or enhance either your job individually, the individuals served, or the collective functions of the Agency.

3. Your personal effort is required in order to serve the Agency’s guiding principle of “...will utilize sound fiscal principles that stress cost containment”. This will be accomplished by your efforts to control overtime expense, turnover of direct care staff, reduction in replacement costs of home inventories due to theft/misuse/mismanagement, and a dedication to safety and quality control.

4. You will adhere to an acceptable level of professionalism to insure the maintenance of confidentiality and discretion necessary in the maintenance of client and personnel records.

5. You will observe all the Agency’s routine regarding work schedules, presence during working hours which conforms to those of the Center, designated periods of break time, lunch hour, etc.

6. You will strive to perform all work assignments and activities in an acceptable manner to require minimal supervision.

7. You will perform any other duties as assigned by the Chief Operating Officer, ECF Program Director, Senior Director of Day services or Senior Director of Residential Services

8. You must establish and maintain good working relationships with coworkers and clients.

9. Your regular attendance is a job requirement; more than 1 unplanned absence in a 2-month period is considered excessive.
