Job description

Your future role at a glance

Location: Nashville, TN

Facility: Saint Thomas Midtown

Department: Case Management

Schedule: Full-Time | Days

How you'll make an impact in this role
  • Team Leadership: Manage assigned case managers, professional associates, and clerical staff to ensure productive workflows. 
  • Operational Oversight: Coordinate disease management programs and track patient outcomes throughout the post-acute period. 
  • Resource Management: Contract with internal and external healthcare resources to secure seamless transitions of care. 
  • Interdisciplinary Collaboration: Communicate regularly with administration and medical staff regarding admission appropriateness, length of stay (LOS), and discharge barriers.
What minimum qualifications you'll need

Licensure / Certification / Registration:

One or more of the following required:

  • Registered Nurse credentialed from the Tennessee Board of Nursing obtained prior to hire date or job transfer date.
  • Social Worker credentialed from the Tennessee Board of Social Workers obtained prior to hire date or job transfer date.

One or more of the following preferred:

  • Case Manager credentialed from the American Case Management Association (ACMA).
  • Case Manager credentialed from the Commission for Case Manager Certification (CCMC).

Education:

  • High school diploma/GED with 2 years of experience, or Associate's degree, or Technical degree required.
    • Master's degree preferred.

Work Experience:

  • 3 years of experience required.
  • 1 year of leadership or management experience required.
