Job description

Position Summary:
We are seeking actively licensed Nurse Practitioners for 1099 Independent Contractor opportunities to perform HCC Wellness Assessments. This flexible role provides an excellent way to earn supplemental income while delivering high-quality preventive care to senior patients.
Compensation: $90 for telehealth assessments and $150 for in -person wellness assessments
Essential Duties and Responsibilities include the following:
  • Provides anticipatory guidance, screening, and health education to patients to promote and maintain health.
  • Provides annual health assessments for senior patients in a variety of settings, such as in our Senior Wellness Clinic, in various provider offices, and at the patient's home.
  • Assesses senior members for preventative interventions such as safety, fall risk, depression screen, and urinary concerns.
  • Evaluates the chronic conditions of senior structures individuals, and/or families or groups about health or defined health problems.
  • Provides education to patients and family members based on screenings, assessments, and the patient's chronic conditions
  • Plans appropriate care based on screening and chronic care guidelines.
  • Plans appropriate care utilizing community resources and acts as a liaison by relaying information necessary for continuity of care to other health professionals and agencies.
  • Flexible with the setting of the evaluations based on the company's needs.
  • Promotes guest relations; anticipates and responds to the needs of others; assists in a courteous, quick manner; treats others with care and respect, while maintaining privacy, confidentiality, and dignity.
  • Maintains a professional appearance and a clean, safe environment. Participates with the company's Quality and Coding departments to ensure the requirements established by the Centers for Medicare and Medicaid Services (CMS) are fulfilled for each senior patient.
  • Provides clinical services by managing patients in the home or clinic.
Employer will consider for employment qualified applicants with criminal histories in a manner consistent with the requirements of the LA City Fair Chance Initiative for Hiring Ordinance.
Education and / or Experience:
  • Active California Nurse Practitioner License
  • Active California RN License
  • Current BLS certification
  • Valid California Driver's License and proof of insurance.
  • HCC experience
  • Demonstrated proficiency in the use of Microsoft Office, Word, Excel, and other applications as required
  • Excellent observational, verbal, and written communication skills
  • Interpersonal skills necessary to work productively with patients, families, nursing staff, and other departments
  • Bilingual, Spanish speaking helpful
  • Three to five years of experience as a Nurse Practitioner - Preferred
