Position Summary
Higher Ground IHS is seeking a knowledgeable, organized, and compassionate Agency Broker to support individuals enrolled in New York State OPWDD Self-Direction services. The Agency Broker will assist individuals and families with developing, implementing, and maintaining Self-Direction budgets and services while ensuring compliance with OPWDD regulations and person-centered planning principles.
This role requires strong communication skills, attention to detail, advocacy, and the ability to work collaboratively with families, care managers, Fiscal Intermediaries, and community providers.
Key Responsibilities
  • Assist individuals and families in understanding OPWDD Self-Direction services and options
  • Develop and maintain person-centered Self-Direction budgets
  • Support completion and submission of Self-Direction documentation
  • Assist with staffing plans, community supports, and individualized goals
  • Coordinate with Care Managers, Fiscal Intermediaries, brokers, and providers
  • Monitor budget utilization and service implementation
  • Ensure ongoing compliance with OPWDD guidelines and timelines
  • Maintain accurate and timely documentation and case notes
  • Support participants with community integration and resource navigation
  • Attend Life Plan meetings and interdisciplinary team meetings as needed
  • Educate families regarding Self-Direction rules, flexibility, and responsibilities
Qualifications
Preferred:
  • Experience working with OPWDD Self-Direction services
  • Knowledge of OPWDD regulations and Medicaid Waiver services
  • Experience with person-centered planning
  • Strong organizational and communication skills
  • Ability to manage multiple cases and deadlines
  • Proficiency in Microsoft Office and electronic documentation systems
Minimum Requirements:
  • High School Diploma or GED required
  • Associates or Bachelors degree in Human Services, Social Work, Psychology, or related field preferred
  • Valid NYS Drivers License and reliable transportation
  • Ability to travel locally within the Capital Region
  • Background check and required clearances
