Schedule: Part-Time โ€“ 20 hours per week

A Place Called Home LLC is a Minnesota 245D licensed provider dedicated to empowering individuals with disabilities and behavioral health needs to live independently in their communities. We are currently seeking a compassionate, dependable, and motivated Individualized Home Supports (IHS) Practitioner to provide person-centered support to individuals in the Prior Lake and Belle Plaine areas.

Job Responsibilities

As an IHS Practitioner, you will provide individualized support based on each personโ€™s unique goals and support plan. Responsibilities may include:
  • Assisting individuals with developing and maintaining independent living skills.
  • Supporting community integration and participation in meaningful activities.
  • Assisting with meal planning, grocery shopping, and meal preparation.
  • Providing homemaking support, including light housekeeping and laundry.
  • Assisting with scheduling and attending appointments as needed.
  • Supporting medication reminders (when appropriate).
  • Encouraging skill development, self-advocacy, and independence.
  • Completing required documentation and EVV accurately and on time.
  • Communicating regularly with supervisors and members of the individualโ€™s support team.

Qualifications

Required

  • High school diploma or GED.
  • Valid driverโ€™s license and reliable transportation.
  • Current automobile insurance.
  • Ability to pass a DHS background study.
  • Strong communication and organizational skills.
  • Ability to work independently while maintaining professionalism.
Preferred

  • Experience working with individuals with disabilities, mental health needs, or older adults.
  • Experience providing HCBS or 245D services.
  • CPR/First Aid certification (or willingness to obtain).
