Job description

The Housing First Case Manager creates permanent housing opportunities by developing an inventory of available housing units for IHS and other program (Shelter Plus Care, TANF, HPRP, etc.) clients with varying price ranges. Provides short term case management and/or work with other case management staff to ensure client's independent living skills help to maintain landlord/rental management relations.
Essential Functions

  • Implements IHS Supportive Housing programs to achieve performance outcomes of each program.
  • Provides outreach and establishes relationships with landlords to market housing programs.
  • Inspects rental units and communicates housing quality to HUD, landlord and client.
  • Conducts face to face housing presentations and schedules housing interviews with clients.
  • Assists clients with their service needs by maintaining a warm and welcoming attitude throughout the service episode.
  • Coordinates clients' applications to subsidy programs such as Section 8, or other types of permanent housing subsidies and assists them in meeting tenancy requirements, i.e. payments and lease requirements.
  • Handles evictions and lease compliance issues.
  • Assesses functioning level of clients' independent living skills.
  • Develops janitorial, maintenance and preventive plans that include regular checking to uncover problems to avoid crisis or failed placement.
  • Develops on-site behavior training including tenant safety education.
  • Provides internal reports of program activity as well as administer standardized client self-report measures as required by program specification and as determined by the Housing & Employment Director
  • Maintains records of referrals, assessments and placement plans in compliance with HUD Housing Counseling Plan.
  • Provides on-going evaluation, including Housing Assessment.
  • Documents all activities and interactions with clients through detailed progress/case notes.
