Job description

Job Description:Division: Permanent Supportive Housing

Essential Functions

  • Provide individualized housing stabilization services to help residents maintain successful tenancies and prevent eviction.

  • Develop and implement Housing Retention Plans, Behavioral Agreements, housekeeping plans, and rent repayment plans.

  • Conduct monthly home visits to assess housing conditions, reinforce lease compliance, and identify tenancy risks.

  • Coach residents on independent living skills, including budgeting, housekeeping, lease responsibilities, conflict resolution, and tenant rights.

  • Utilize trauma-informed care, Housing First, motivational interviewing, and harm reduction approaches to engage residents and promote positive outcomes.

  • Partner with Property Management to address lease violations, coordinate interventions, and support housing retention efforts.

  • Maintain timely and accurate documentation, including case notes, home visit records, and housing stabilization plans.

  • Monitor resident progress, track housing stability outcomes, and assist with program reporting.

  • Collaborate with Case Managers, Clinical staff, Property Management, and community partners to coordinate resident services.

  • Participate in team meetings, case conferences, required trainings, and continuous quality improvement initiatives.

Qualifications

Education

  • High school diploma or GED with two (2) years of related experience; or

  • Associate's or Bachelor's degree in Human Services, Social Work, Psychology, Sociology, or a related field.

Experience

  • Two (2) years of experience working with individuals experiencing homelessness, mental illness, substance use disorders, or other barriers to housing.

  • Experience in Permanent Supportive Housing (PSH), Housing First, Rapid Re-Housing, or supportive housing programs preferred.

  • Knowledge of Fair Housing, lease compliance, eviction prevention, and housing retention strategies preferred.

Knowledge, Skills & Abilities

  • Strong communication, relationship-building, and problem-solving skills.

  • Ability to work independently and as part of a multidisciplinary team.

  • Experience with trauma-informed care, motivational interviewing, and strengths-based practices preferred.

  • Proficient in Microsoft Office and electronic documentation systems.

  • Ability to maintain accurate documentation and manage multiple priorities.

  • Bilingual (Spanish, ASL, or other languages) preferred.
