James River Hospice is actively seeking a compassionate and highly committed Hospice RN Case Manager to join our growing team! You will be responsible for the hospice case management of your assigned patients on an intermittent basis in their place of residence. You will also coordinate with the interdisciplinary team to ensure an optimal care experience for the patient!

Brief Overview of Responsibilities for Hospice Registered Nurse:
  • Complete an initial assessment of the patient and family to determine hospice needs
  • Develops a care plan that establishes goals based on nursing diagnosis and incorporates palliative nursing actions.
  • Provides coordination among the clinical team supporting the patient and supervising LPNs and Hospice aides who may be involved in patient care.
  • Spend quality time with the patient to assist with pain management and support
  • Modify and re-evaluate the plan of care when necessary
  • Monitor, record, and report symptoms and changes in the patient's condition
  • Obtain and receive physician orders as required
  • Participates in on-call duties as defined by the on-call policy
  • Other duties as assigned

Qualification of Registered Nurses:
  • Must be a Registered Nurse with a current license to practice nursing in the Commonwealth of Virginia
  • At least one year of experience in hospice care is required.
  • Graduate of an accredited nursing school.
  • Must have a valid driver's license and a reliable car available for use during work hours.
