About the position:

The Registered Nurse (RN) Case Manager coordinates all aspects of the patientโ€™s home care. You will interact with other medical disciplines as needed & provide professional care to the patient. You will also perform the admission visit to the patient in the home, determine the patientโ€™s eligibility for home care services, & develop the plan of care to be followed.

Qualifications:
  • Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
  • Current state license as a Registered Nurse.
  • Current state Driverโ€™s License.
  • One-yearโ€™s experience as a Registered Nurse, two preferred.
  • Proof of current CPR and Hepatitis consent/declination.
  • Reliable transportation with valid and current auto liability insurance.
Pay Range:
  • $74.00 โ€“ $90.00 per hour (Full-Time)
  • $31.00 โ€“ $40.00 per hour, $60.00 โ€“ $110.00 per visit* (PRN)
