Job description

Sentara is looking for a Part Time Hospice Advance Practice Provider for Chesapeake Hospice. The APP will be responsible for in home visits, telehealth appointments and returning patient phone calls. There will also be Teams and In person office meetings. This is a Hybrid position.

Hours- Any 3 days--Monday- Friday 8am-5pm (24 hours per week)

The Nurse Practitioner (NP) is responsible for the provision of health care services to individuals, families, and/or groups who receive their care under the auspices of Sentara. Health care services include conducting health assessments, prescribing/providing treatments/interventions, promoting health, and providing disease prevention and management. Health care services are provided under the direction and supervision of assigned primary care physician (collaborative agreement). The Nurse Practitioner is expected to participate in quality assurance and improvement activities, engage in team activities, and use resources (human and material) appropriately.

Education

Master's Level Degree- Nurse Practitioner

Certification/Licensure

One of the following certifications qualifies:

Registered Nurse (RN) Multi State - Nursing License - North Carolina Board of Nursing (NCBON)

OR

Registered Nurse (RN) Single State - Nursing License - North Carolina Board of Nursing (NCBON)

OR

Registered Nurse (RN) Multi State - Nursing License - Virginia Department of Health Professionals (VADHP)

OR

Registered Nurse (RN) Single State - Nursing License - Virginia Department of Health Professionals (VADHP)

AND

One of the following certifications qualifies:

Advanced Practitioner Registered Nurse (APRN) - Nursing License - Virginia Department of Health Professionals (VADHP)

OR

Licensed Nurse Practitioner (NP) - Nursing License - North Carolina Board of Nursing (NCBON)

BLS (Required)

ACLS (Preferred)

Requirements

Nurse Practitioner experience preferred in Hospice or Palliative Care, Oncology, Geriatrics or Family Practice
