Job description

Hospice Nurse- RN (Open)

The RN Hospice provides end-of-life nursing care, coordinates symptom management and hospice services, collaborates with the interdisciplinary team, and delivers compassionate care to patients and families across care settings.

RN Hospice Nurse. Mon-Thurs on call. Mileage reimbursement. Company equipment. Sign on bonus eligible.

Responsibilities include:

  • Serving as a member of the hospice team, completing admissions, coordinating care, and conducting follow-up visits.
  • Reviewing patient eligibility and collaborating with the interdisciplinary team, Medical Director, and attending physician to develop care plans.
  • Providing compassionate, patient-centered care to individuals with terminal illness across care settings.
  • Performing comprehensive assessments and developing individualized plans of care.
  • Administering treatments and medications accurately and documenting care in electronic medical records.
  • Advocating for patients and supporting their rights and psychosocial, cultural, and emotional needs.
  • Participating in interdisciplinary team meetings and collaborating across care teams.
  • Contributing to performance improvement initiatives and supporting positive patient outcomes.
  • Utilizing clinical expertise in dynamic and changing patient conditions.
  • Implementing strategies to improve care efficiency, outcomes, and resource utilization.
  • Serving as a role model and providing support to staff, students, and team members.
  • Using problem-solving skills to address patient care and operational challenges.
  • Providing feedback to staff and participating in shared decision-making activities.
  • Maintaining strong critical thinking and organizational skills to manage competing priorities.

Compensation Range: $76,689.60 - $115,044.80

Employee pay is based on a variety of factors, including qualifications, credentials, education, and prior work experience. The compensation range does not include benefits or pension plan.
