We offer unique opportunities for Case Managers in several specialized roles, including Early Intervention - Part C, Early Intervention and ID/DD Case Management. Whether your passion is helping young children reach developmental milestones or empowering individuals with ID/DD to achieve their goals, WTCSB provides the support and resources you need to excel.
Responsibilities and Duties for ID/DD Case Manager:
  • Responsible for consumer assessment, service linkage, coordination, development, and implementation of a case management ISP
  • Assist and provide follow-up instruction, education, and counseling to consumers/families with ID/DD.
  • Monitor service delivery through contacts with consumers receiving services, family members, service providers, and periodic site/home visits.
  • Manage consumer records according to all Licensure and Medicaid standards.

Qualifications and Skills:
  • ID/DD experience is required for this position.
  • BA/BS from an accredited college or university with major course experience in Psychology, Social Work, or related field
  • One year of clinical experience in a Human Services field
  • QMHP or QIDP preferred
  • Valid Driver's License with an acceptable DMV record at the time of employment and continuously thereafter
