Job description

Provide physical therapy services to clients in their home, under the direction of a physical therapist.

Work Schedule: 1.0 FTE; M-F 8-4:30pm

This position will be covering our Wisconsin side, which is the western part of St. Croix County- northern part of River Falls, Hudson, Hammond, Baldwin, New Richmond, Somerset, Deer Park, Houlton, etc.; paid time to drive and paid mileage.

Required Qualifications:

  • Graduate of accredited physical therapy assistant program
  • Continuing education as required by state licensing boards
  • Must have MN or WI Physical Therapy Assistant (PTA) license upon start date.
  • Must provide proof of MN and WI Physical Therapy Assistant (PTA) license within 3 months of start, if both MN and WI Physical Therapy Assistant (PTA) licenses are not present on start date
  • Must have a vehicle with auto insurance and a valid driver's license and meet all criteria required under Lakeview's Automobile Liability Guidelines
  • Current BLS - American Heart Association or BLS - American Red Cross (CPR/AED for Professional Rescuer & Health Care Provider)
  • Ability to read, write, and speak the English language

Preferred Qualifications:

  • Proof of application for alternate state within 4 weeks of start, if both MN and WI Physical Therapy Assistant (PTA) licenses are not present on start date
