Clinical Scheduler - Home Health
Clinical Scheduler Responsibilities
  • Coordinate and maintain daily and weekly Home Health schedules for Registered Nurses (RNs), Physical Therapists (PTs), Occupational Therapists (OTs), and Clinical Assistants (CAs).
  • Schedule and coordinate all required patient visits, including Start of Care (SOC), Resumption of Care (ROC), Recertifications, Discharges, Evaluations, and routine Skilled Visits.
  • Ensure SOCs and other time-sensitive visits are scheduled within required timeframes and assigned to the appropriate clinician.
  • Schedule ongoing skilled nursing and therapy visits according to the patient's plan of care, ordered frequency, clinical needs, and clinician availability.
  • Coordinate therapy evaluations and ongoing PT and OT visits while ensuring patients are assigned to the appropriate clinical discipline.
  • Manage clinician schedules based on geographic territory, patient location, clinician availability, caseload, productivity expectations, and scope of practice.
  • Work closely with clinical leadership to appropriately utilize Clinical Assistants and ensure licensed clinicians are working at the top of their licensure whenever possible.
  • Monitor schedules throughout the day and proactively address call-offs, missed visits, declined visits, patient cancellations, hospitalizations, schedule changes, and urgent coverage needs.
  • Track open and unscheduled visits to ensure required visits are completed and prevent gaps or delays in patient care.
  • Review upcoming schedules to identify SOC, Recertification, ROC, discharge, and skilled visit coverage needs before they become scheduling concerns.
  • Communicate schedule assignments and changes promptly with clinicians, patients, caregivers, and clinical leadership.
  • Partner with Nursing, Therapy, Intake, and Clinical Operations to coordinate new admissions and ongoing patient care.
  • Monitor clinician capacity and communicate staffing or coverage gaps that may impact the agency's ability to accept new patients or meet current patient needs.
  • Maintain accurate patient and clinician scheduling information within the company's EMR and scheduling systems.
  • Support efficient clinician utilization by minimizing unnecessary travel and grouping patient visits geographically whenever possible.
  • Maintain patient confidentiality and comply with HIPAA, company policies, and applicable Home Health requirements.
  • Perform additional scheduling and administrative responsibilities as needed to support Home Health clinical operations.
Preferred Qualifications
  • 1+ year of healthcare scheduling, staffing, or coordination experience required; Home Health experience strongly preferred.
  • Understanding of Home Health visit types, including SOC, ROC, Recertification, Discharge, Evaluations, and Skilled Visits preferred.
  • Experience scheduling nursing and therapy clinicians strongly preferred.
  • Familiarity with physician orders, visit frequencies, plans of care, and Home Health scheduling workflows preferred.
  • Strong organizational skills with the ability to manage a high-volume and frequently changing clinical schedule.
  • Ability to prioritize time-sensitive visits and quickly resolve coverage issues.
  • Strong critical-thinking and problem-solving skills.
  • Excellent communication skills when working with clinicians, patients, families, and leadership.
  • Strong attention to detail and ability to track multiple patients, clinicians, disciplines, and visit requirements simultaneously.
  • Experience working within a Home Health EMR or scheduling platform preferred.
  • Ability to work effectively in a fast-paced environment where patient and clinician needs may change throughout the day.
