Job description

The Clinical Educator, Home-based Care, is responsible for the clinical education of all home-based services. The Clinical Educator establishes, implements, and evaluates educational goals and objectives for home-based care services to exceed the standards of quality and contribute to the Integritus mission and philosophy. The Clinical Educator collaborates with Executive Directors, Branch Managers, Quality, and Human Resources to provide a process to promote and ensure employee orientation, competency, and continuing education programs, including preparation for certification exams.
Essential Job Functions
  • Oversees the development and implementation of the Integritus Home-based care annual Staff Education Plan (identify and schedule subjects/trainers/Relias learning, ensure that records are maintained for attendance, competency, and evaluations, ensure plan meets compliance requirements, provide training as directed).
  • Collaborates with Quality and other leaders as needed to develop tools to support QAPI projects, including mandatory in-service programs.
  • Identifies skills for annual competence, provides in-service training, and support.
  • Collaborates with Human Resources and the Executive Director to provide orientation, including field visits to ensure competency.
  • Collaborates with Human Resources to maintain updated information on certifications and supervisory/competency regulations.
  • Provides support for staff regarding electronic documentation, to ensure documentation is completed, documented, and implemented in a timely and appropriate manner.
  • Participates actively in staff orientations, training, and annual policy reviews.
  • Oversees the development, implementation, and evaluation of the orientation program. Ensures orientation of new staff directly or as delegated.
  • Participates in Administrator On-call rotation.
  • Maintains a thorough knowledge of Hospice and Home-Health Regulations and Standards. Responsible for complying with and promoting accepted professional standards and principles.
  • Demonstrates excellent and respectful communication skills with the care team.
  • Assists in the development of organizational education goals. Develops, recommends, and administers organization education policies and procedures.
  • Stays informed about changes in home-based care and shares regularly with home health, hospice, and palliative care personnel.
  • Ensures proper maintenance of the clinical record in compliance with local, state, and federal laws.
  • Responsible for maintaining up-to-date nursing knowledge and acts as a clinical resource to staff.
  • Communicates staff compliance with educational programs to supervisors.
  • Makes ride-along visits as needed to assist staff with competency, documentation, and case management skills.
  • Responsible for ensuring processes to plan, implement, and educate on the infection control program.
  • Collaborates with agency Managers/Coordinators to promote and ensure employee/volunteer orientation, competency, and in-service education programs related to infection control practices.
  • Assists in the development of written standards for clinical practice documentation
  • Assists with the admission of hospice patients.
