House Call Nurse Practitioner

Brooklyn, NY

Full-time
Deliver personalized home-based primary care from a Brooklyn hub, focusing on homebound patients throughout NYC.

Position Overview

A large multispecialty group in NYC is seeking a Nurse Practitioner (NP) to provide high-quality, patient-centered care in patients’ homes and nursing facilities. This role is ideal for providers who enjoy holistic care, chronic disease management, and direct patient engagement.

Emergency Response & Community Engagement

· Assess and manage urgent medical issues during visits

· Participate in community outreach and patient education initiatives


Qualifications

· Master’s Degree from an accredited NP program (Family or Adult/Geriatrics focus)

· Current New York State NP license

· National Board Certification (AANP, ANCC, or ONCC)

· Due to patient population, client is seeking providers who are bilingual in Russian or Chinese

· Experience with EMR/EHR systems preferred

· Strong clinical judgment, communication skills, and ability to work independently
