Job description

*This position is Full Time, Non-exempt Hourly; Starting Range $20.20 to $22.72; Day (8am-5pm) and Night Shifts (1pm-9:30pm) 

The Grant Case Manager is responsible for tracking unsheltered guests’ progress through case management and ensuring the effective administration of various grant programs. Ensure timely documentation of services (including but not limited to case files, client outcomes, and agency technical reports) and timely data input into applicable system. Work within the Housing First best practices and in collaboration with the Continuum of Care (CofC) and Consolidated Entry System (CES). As this position is funded by grant resources, its continuation is dependent on the availability of grant funding. Given our successful track record, we anticipate the acquisition of additional future grants.

Provide comprehensive case management for unsheltered guests eligible for financial assistance through the various grants. Also provide case management for unsheltered guests who are housed in our Interim & Permanent Housing programs and those seeking emergency shelter. Conduct initial intake in coordination with United Way 211 via the CES, when applicable. Establish and maintain comprehensive case files, including guests’ Individual Housing Action Plan (IHAP) and required documentation for income eligibility and financial assistance. Identify and coordinate wrap-around support services necessary for guest stabilization and sustainability. Track guest goals, milestones and outcomes, which will require conducting home visits. Ensure the privacy and security of all guest records containing Personal Identifiable Information (PII). Answer emails and calls, maintain a calendar of events, and ensure accurate and up-to-date record-keeping for grant monitoring. Assist in preparing materials for various programs using software such as Microsoft Word, PowerPoint, Excel and Publisher. Uphold the current policies, procedures, and best practices for intake and service distribution across all programs. 

Enter guest information accurately into the County HMIS system and verify entries through the production of technical reports.

Collaborate with the Street Outreach Specialist, Housing Resource Specialist, and CAP Coordinator to leverage resources for guests seeking additional supportive services. Assist with class scheduling, set-up and record keeping. Attend case conferencing meetings and participate in CityLink outreach events that support wrap-around services such as: CEO’s in the Park, Holiday Food Baskets and Christmas Toy Store. Perform other duties as assigned. 

Participate in “All-In” events, Prayer Mountain, Spiritual Growth Check List, etc.
