Description:

The part-time Senior Case Manager will a ssists students towards self-sufficiency through accountability, mentoring, instruction, and coordination of services.


This role is scheduled Monday, Wednesday, Thursday, and Friday from 3:00 p.m. to 8:00 p.m., and Tuesday from 1:00 p.m. to 6:00 p.m.

Essential Job Duties with optional subcategories (Please add time spent in percentage for each job duty):

  1. Create, implement and execute Individualized Treatment Plans in collaboration with students.
  2. Meet weekly with each assigned student to assess progress of ITP and ensure program compliance.
  3. Ensure documentation such as case notes, written warnings and goal tracking reports are clear and timely.
  4. Participate fully in implementing program changes and directives from management.
  5. Provide both practical and biblical assignments that will lead towards growth in struggling areas.

6. Teach new life skills and coping skills, so that students can replace bad behavior with good behavior.

7. Assign appropriate wrap around services such as groups, classes, therapy, law clinic, health clinic, etc.

8. Provide appropriate disciplinary actions up to and including dismissal from program as needed.

9. 1on1s and counseling sessions

10. Collaborates with outside agency partners such as probation officers, social workers, clinic staff, etc. to support students ITP.

11. Must be flexible and a team player.

12. Help students budget money and equip them with life skills

13. Ensure they are participating in life groups or recovery groups and a member of a church before completion of their program

14. Hold students accountable to VOH guidelines

Key Performance Indicators (what are the main job duties performed that measure success for this position and organization goals): To an extent, key performance indicators will be whether students are progressing to the next phase. What achievements have been made in the students ITP. Accuracy and timeliness of case notes and reporting.


Job Skills/Qualifications: Bachelorโ€™s Degree or equivalent education and experience in a related field such as in a social work/counseling environment or within a ministry setting. Knowledge and experience working with men, women and children in crisis. Strong working knowledge of the Bible. Strong verbal and written communication, organizational and administrative skills. Christian Association of Drug and Alcohol Counselor (CADAC) or Registered Addictions Specialist (RAS) certification desired

Requirements:

Part II โ€“ Job Requirements

Education: Bachelor's degree (BA)

Experience: 1 year of job related work experience.

Communication Skills: Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups.

Math Skills: Minimum Skills: Ability to add and subtract and to multiply and divide.

Reasoning: Intermediate Skills: Ability to apply common sense understanding to carry out instructions furnished in written, oral or diagram form. Ability to deal with problems involving several variables in standarized situations.
