Job description

Job Title: Data Analyst and Aftercare Coordinator
FLSA Status: Full Time - non-exempt
Reports to: Program Director
Schedule: 8:30 - 5:00 PM
Supervises: N/A
Rate of Pay: $30.00-$31.25 hourly
Closing Date: 8/28/2026
Note: This position is based on-site in Denver, CO and requires the ability to reliably commute to our downtown location.
Benefits: Standard; Full-Time, Non-Exempt employees are eligible for but not limited to the following:

  • Sick leave benefit - 1 day per month, 12 sick days per year (accrual and availability begins at hire)

Function
The Harbor Light Center-a men's relapse-prevention and transitional living program dedicated to long-term resident success-is seeking a Data-Analyst & Aftercare Coordinator. This role is responsible for developing, tracking, and improving program outcomes across Harbor Light, with a strong focus on aftercare, alumni engagement, community engagement, and data integrity. The Specialist supports current and past program participants, including veterans, and plays a key role in enhancing program effectiveness, retention, and long-term recovery outcomes. This position reports directly to the Program Director.
Duties and Responsibilities
Program Outcomes & Data Management
  • Develop systems to collect, analyze, and report program data (housing stability, sobriety milestones, employment, aftercare participation, recidivism rates, etc.).
  • Identify gaps, trends, and areas of concern in program outcomes, and collaborate with leadership to propose solutions.
  • Maintain accurate, timely, and detailed outcome records for all active and alumni participants, including VA required data.
  • Manage outcome tracking platforms such as HMIS, Wellsky, and other internal databases; ensure data integrity and compliance.
  • Produce monthly and quarterly program outcome reports for leadership, grant requirements, fundraising use, and quality-improvement initiatives.
  • Maintain the entire program census, including reports required by the VA, other grants, and internal stakeholders.
  • Provide backup HMIS support to the Intake Coordinator and ensure seamless transition from intake through discharge and aftercare.

Aftercare & Alumni Engagement
  • Work with the Program Director and Program Manager to develop a robust aftercare program that supports graduates up to one-year post-discharge.
  • Implement a structured follow-up schedule (30, 60, 90 days, 6 months, 12 months) to assess stability, identify relapse risks, and connect alumni to ongoing resources.
  • Maintain consistent contact with graduates to increase success rates, using progressive engagement strategies.
  • Organize quarterly alumni events, peer-support gatherings, and community-building opportunities.
  • Track participation in aftercare services and evaluate the effectiveness of engagement strategies.
  • Manage a small caseload of graduates for ongoing support and follow-up.

Housing & Community Resource Support
  • Collaborate with external agencies, landlords, and housing partners to secure stable housing opportunities for residents.
  • Maintain updated housing and community resource materials for residents, alumni, and case managers.
  • Provide housing and aftercare support to veterans in accordance with VA contract requirements.

Community Engagement & Volunteering
  • Recruit and coordinate volunteers or partner organizations to enhance aftercare supports and alumni engagement.
  • Develop referral feedback loops to understand the success of external placements and improve collaborative outcomes.

Program Support & Education
  • Teach classes focused on post-graduation success a minimum of 1 hour per week.
  • Assist with enforcing program rules and maintaining a positive, therapeutic community environment.
  • Attend all staff meetings and participate in interdisciplinary collaboration.

General Expectations
  • Maintain professional and confidential relationships in all interactions.
  • Comply with all Salvation Army policies and procedures.
  • Perform other duties as assigned to ensure seamless program operations.

Education, Experience, Skills, Qualifications
• Bachelor's degree in Human Services, Social Work, Counseling, Public Health, Data Analytics, or related field (required). Master's degree preferred.
• Experience working in recovery, residential programs, transitional housing, or behavioral health settings.
• Demonstrated experience with data collection, tracking, and outcome reporting (HMIS experience strongly preferred).
• Strong analytical skills with the ability to identify patterns, gaps, and opportunities for improvement.
• Ability to build rapport with residents, alumni, and community partners.
• Strong organizational, documentation, and communication skills.
• Ability to manage multiple projects, deadlines, and follow-up schedules.
• Knowledge of VA-specific program requirements is a plus.
• Must be able to work collaboratively within a team and independently with minimal supervision.
• Cultural competence and ability to work with diverse populations.
Driving
• Valid driver's license required.
• A minimum age of 21 (for insurance provision) and possession of a valid in-state driver's license is required.
• An MVR will be processed every year in accordance with The Salvation Army's policies.
Background Check
• Continued employment will be contingent upon a biennial (every two years) background check that is processed in accordance with The Salvation Army's policies.
