Job description

HIV Early Intervention Specialist
Position Summary
The HIV Early Intervention Specialist provides HIV testing, prevention education, outreach, PrEP coordination, patient support, and linkage to medical and community services. This position also provides case management and patient care services under the direct supervision of the Chief Medical Officer.

South County Mental Health Center, Inc. is a private, non-profit, Joint Commission-accredited behavioral healthcare provider serving Southeast Palm Beach County. We are dedicated to delivering compassionate, high-quality mental health services to individuals and families in our community.
Minimum Qualifications
  • Florida Department of Health (Palm Beach County) HIV/AIDS 500/501 Certification. 
  • Minimum one (1) year of experience in a human services field providing outreach and supportive case management services to at-risk, underserved, indigent, and/or homeless populations. 
  • Active Florida Phlebotomy Certification. 
  • Ability to work effectively with diverse and vulnerable populations.
  • Strong verbal and written communication skills.
  • Proficiency with Microsoft Office.

Preferred Qualifications:
  • LPN, RN, or Therapist.
  • Experience working with individuals living with HIV or populations at high risk for HIV infection. 
  • Experience with HIV prevention, PrEP, outreach, or community-based services.
  • Experience with electronic health record documentation. 

Duties and Responsibilities
HIV Prevention, Testing & Outreach
  • Provide HIV and other testing, counseling, education, and referral services.
  • Provide education and prevention resources related to sexually transmitted infections.
  • Participate in community events, outreach activities, and collaborative agency initiatives.
  • Educate clients regarding HIV prevention, diagnosis, medications, PrEP, treatment, and testing. 
PrEP Coordination & Patient Support
  • Schedule initial and ongoing PrEP appointments.
  • Facilitate telehealth appointments with the PrEP provider.
  • Schedule follow-up appointments for clients.
  • Assist patients with accessing PrEP services and completing required documentation.
  • Perform pre-visit blood draws for PrEP laboratory services. 
Care Coordination & Case Management
  • Provide supportive case management and patient care services.
  • Assist with referrals to appropriate internal and external service providers.
  • Obtain required releases, consents, and health information.
  • Assist with procedures, referrals, and patient follow-up as needed.
  • Promote continuity of care and access to appropriate services. 
Documentation & Data Management
  • Accurately document patient health history and vital signs in the EMR.
  • Maintain accurate documentation of HIV testing, patient education, and PrEP-related services.
  • Collect, track, and report HIV/AIDS and other testing data as required.
  • Ensure required HIV testing is maintained and documented according to program requirements. 
Team Collaboration
  • Provide support to physicians, nurse practitioners, therapists, behavioral health providers, and support staff involved in HIV care.
  • Coordinate with internal departments and external community agencies.
  • Participate in meetings, training, and collaborative activities as assigned. 

Core Competencies
  • Knowledge of HIV prevention, testing, PrEP, and STI prevention.
  • Strong patient education and advocacy skills.
  • Ability to work effectively with vulnerable populations, including individuals experiencing trauma, poverty, homelessness, substance use, incarceration, serious mental illness, and housing instability. 
  • Ability to multitask and work effectively in a fast-paced environment.
  • Strong organizational and problem-solving skills.
  • Ability to maintain confidentiality and comply with HIPAA requirements. 
  • Excellent verbal and written communication skills.
  • Proficiency with Microsoft Office and electronic health records. 

Physical Demands
  • Frequent standing, walking, sitting, and computer use.
  • Ability to perform blood draws and other patient-support duties.
  • Ability to participate in community outreach activities and events.

Schedule & Work Details
Full-Time Position
Schedule: Monday – Friday (hours may vary based on organizational needs)
Location: In-person (Delray Beach, FL)
Pay: $25 per hour Masters/LPN; $40 per hour RN
