Job Summary:
The Aliveness Project of Northwest Indiana is seeking a Crisis Case Manager and Navigator to provide rapid, community-based support to individuals experiencing behavioral health crises, including those involving suicidal ideation, substance use, or other acute distress. Using a harm reduction and trauma-informed approach, the Crisis Case Manager will help stabilize clients, coordinate linkage to services, and ensure continuity of care following crisis intervention.
This role bridges immediate crisis response with long-term support, emphasizing safety, recovery, and empowerment for individuals in crisis within our community.
Essential Duties and Responsibilities

Respond as part of the Mobile Crisis Team to individuals experiencing mental health, substance use, or co-occurring crises in community, residential, or shelter settings.

Conduct risk assessments for suicide, self-harm, or danger to others in collaboration with licensed clinicians.
โข
Provide on-site crisis stabilization, including de-escalation, harm reduction interventions, and safety planning tailored to the client's needs.
โข
Coordinate with law enforcement, hospitals, EMS, shelters, and behavioral health providers to ensure safe transport and effective linkage to ongoing services.

Develop individualized crisis recovery and follow-up plans, ensuring clients are connected to outpatient treatment, case management, primary care, and supportive services.
โข
Conduct post-crisis follow-up within 24-72 hours to confirm client safety, assess ongoing needs, and ensure continuity of care.
โข
Maintain accurate, timely case documentation in the electronic health record (EHR) or client relationship management (CRM) software to ensure compliance with all agencies, state, and federal standards.

Participate in multidisciplinary team meetings, case conferences, and quality improvement initiatives.
โข
Assist with data collection and program reporting to support compliance, evaluation, and funding deliverables.
โข
Support community education and outreach to increase awareness of crisis response services and promote harm reduction principles.

Successful completion and understanding of all training programs, including but not limited to Mental Health First Aid, Crisis Intervention Team (CIT), and equivalent crisis training.

Job Requirements and Qualifications

Bachelor's degree in Social Work, Psychology, Human Services, or a related field required. Master's degree preferred.
โข
Minimum two (2) years of experience in case management, crisis intervention, or behavioral health required. Experience with suicide prevention, harm reduction, or substance use programs strongly preferred.

Mental Health First Aid, Crisis Intervention Team (CIT), or equivalent crisis training preferred.
โข
Valid driver's license, clean driving record, and reliable transportation required.
โข
Strong understanding of harm reduction, trauma-informed, and recovery-oriented models of care.
โข
Demonstrated ability to engage and de-escalate individuals in acute distress effectively.
โข
Competency in completing risk assessments, developing safety plans, and coordinating care.
โข
Proficiency in electronic documentation (EHR) and data management.
โข
Ability to work independently and collaboratively within a multidisciplinary team.

Working Conditions:

12-hour shift schedule required to ensure 24/7 coverage of the Mobile Crisis Unit.

Shifts may include days, nights, weekends, and holidays on a rotating basis.

Field-based work includes community response and collaboration with partner agencies.

Exposure to emotionally intense situations and unpredictable environments may occur; safety protocols and team response models are always maintained.

Must be able to sit, stand, and drive for extended periods, and respond to community locations as needed.
