Physical Therapist- PT Home Health, Tucson, AZ - Local or Travel

13 weeks.

M-F, 8am - 5pm, 40 hours per week.

On-call info: On-call required if requested- on-call needs are subject to change based on staff and branch needs
Productivity Units: 30 units
Qualifications: HCHB, Oasis, and HH experience required

Skills:
Current State License as a Physical Therapist - Required.
Proof of current CPR certification - Required.
Current driver's license. Must have reliable transportation and insurance - Required
1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting - Required
Previous home care experience - Preferred.

HCHB EMR preferred
Education:
Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program - Required
