RN, Care Manager – Home Health (Contract)

Location: California 95422
Pay: $2,351 – $2,635/week
Schedule: Day Shift, 8-hour shifts (40 hrs/week)
Coverage Area: Lake County, CA (mileage reimbursed)


Position Summary & Responsibilities
  • Manage and coordinate home health patient care plans

  • Collaborate with care team and ensure continuity of care

  • Utilize OASIS and HCHB for documentation and compliance


Requirements
  • Active RN license (CA) – Required

  • Home Health experience – Required

  • HCHB software experience – Required

  • OASIS experience – Required

  • BLS (AHA, HS-BLS, or RQI) – Required

  • BSN – Preferred

  • Case Management experience/certification – Preferred
